Job Summary

Under general professional supervision, performs behavioral health case coordination services in the community including planning, organizing, and treatment referral/disposition. Engages patients through community outreach, both independently and with various external partners, to assess patient needs and develop plan for accessing care. Delivers services in accordance with the approved plan of treatment and plan of care. Assists behavioral healthcare providers in understanding significant social and emotional factors related to the patient's behavioral health issues.

Essential Functions:

Engages clients in various community settings and identifies client's behavioral health needs, plans for implementation and/or coordinates care palns through resources acquisition, monitoring, advocacy, collaboration, and evaluation (60%) Coordinates follow-up treatment referrals to various levels of care including outpatient therapy and psychiatry, intensive outpatient programing, long term care and other services as indicated. Identifies potential barriers to accessing care and works with clients to identify solutions and access additional resources to reduce those barriers. (20%) Develops knowledge base of risk factors for psychosocial functioning, wellness and health promotion and provides basic education to patient. Utilizes methods, techniques, and approaches to the delivery of care that are consistent with the patient population and in conformance with the specific care plan developed for each patient. (10%) Completes documentation of care coordination activities and interventions per established standards and guidelines. Prepares and submits data and information to behavioral health supervisor in a timely manner. (10%)

Education: Master's Degree Social Work, Counseling, Public Health, Human Services, or other closely related field required.

Work Experience: 1-3 years experience working in mental health and/or substance use disorders and/or with individuals experiencing homelessness or at-risk of homelessness required.

Knowledge, Skills and Abilities: Knowledge of emotional and social factors that affect patient's health. Excellent interpersonal skills. Skilled in identifying problems/opportunities to improve care. Ability to utilize resources, such as family and community agencies to assist the patient to improve life in the community. Ability to collect key information from the patient, family and the community as part of the care coordination assessment. Ability to communicate to the appropriate staff and other identified supports information about the patient's care plan. Ability to manage time sufficiently to achieve established goals. Proficiency with software applications such as Microsoft Word, Excel, and Outlook.
