Job Summary

The Case Manager for Corrections assesses, counsels, monitors, and provides case management for incarcerated men and women in the City and County of Denver (care and custody of the Denver Sheriff Department). The Correctional Care staff provides clinical care at either the Downtown Detention Center or the Denver County Jail. These environments of care have a higher level of clinical acuity and environmental hazard than inpatient and ambulatory settings based on the nature of the facility. The detention centers employ a high level of security, which impacts daily working conditions, including both personal and institutional safety and clinical operations. Environmental awareness, critical thinking, and decisive decision-making skills are required in the correctional care setting.

Essential Functions:

Conducts assessment interviews using various assessment tools to appraise and interpret a patient's risk factors and needs, such as alcohol/drug use, anger, depression, suicidal ideation, thought disturbance, criminal behavior/delinquency, employment/educational needs, and relationships with family, friends, or other support systems; evaluates a patient's resiliency factors and strengths; and recommends corrective actions, classes, activities, and/or other appropriate services/resources. (20% Develops an individual program plan/contract with a patient including attending support/prevention groups and/or cognitive/behavioral classes, counseling, employment preparation, participating in community service activities, and/or referring patient to community agencies that meet particular individual needs and establishes goals with timelines for completing specific conditions. (20%) Monitors and verifies patients have completed agreed upon goals/activities, meets with patients, family members/guardians, school staff, and/or other services providers to discuss progress, problems, and solutions, and recommends corrective action for patients in non-compliance. (20%) Maintains patient files and records consistent with departmental policies and local, state, and federal rules and regulations. (15%) Serves on committees in order to discuss issues and recommend changes to improve services/facilities for patients and coordinates and attends meetings with community-based partners/groups, city personnel, and other governmental agencies for the purpose of patient prevention efforts. (10%) Prepares and conducts various cognitive/behavioral classes, facilitates groups, and develops the curriculum for individual groups and classes. (10%) By position, utilizes methods, techniques, and approaches to the delivery of care that are consistent with the age group, developmental skills, and mental health issues of the patient and in conformance with a specific care plan tailored to the unique issues/needs of each patient. (5%) Education:

Bachelor's Degree Bachelor's degree in Psych, Sociology, Human Services, Criminal Justice or related field required, or in lieu of bachelor's degree, 1 year of experience. Required Work Experience:

1-3 years Minimum of one year of experience in performing case management including completing comprehensive needs assessments and developing/implementing case plans. Required or 1-3 years Minimum of one year of experience in case work and counseling in areas such as diversion, probation, alcohol and drug abuse, community-based treatment case management, or a closely related area.
