Summary The Telerehabilitation Integration with Community Based Outpatient Clinics (CBOCs) is an innovative healthcare program designed to enhance access to treatment for musculoskeletal impairments in rural and medically underserved areas. This innovative model leverages multiple modes of access, including telemedicine, specifically the use of clinical videoconferencing equipment, to allow healthcare specialists to provide care to our Veteran population. Responsibilities DUTIES: Clinical: Receives and interprets requests or instructions for procedures from the Physical Therapist. Independently plans and implements skilled specialized treatments and applies physical therapy procedures to Veterans under the general supervision of a physical therapist. Independently renders a course of treatment based on the Physical Therapist assessed needs of the Veteran, utilizing that information to collaboratively develop progressive types of physical therapy interventions. Independently develops a multifaceted skilled treatment plan based on a plan of care set forth by evaluating Physical Therapist. Persuades and motivates Veterans and family members to participate in treatment plans and comply with indicated regimens and post-discharge activities. Interacts with the Veteran and family regarding goals for treatment. Assists in the performance of, or independently performs, highly specialized and diversified treatment procedures. Utilizes the full range of skilled interventions including but not limited to therapeutic exercise, therapeutic activities, therapeutic modalities, gait training, transfer training, manual therapy, neuromuscular reeducation, self-care and management training, patient/caregiver education, orthotic and prosthetic training/education and group therapy. Instructs, motivates, and assists Veterans to learn activities of daily living and improve functional activities. Prepares and maintains appropriate equipment and supplies required for these treatments. Correspondence on daily basis, in addition to general supervision, through telemedicine equipment with evaluating therapy team. Daily huddles to address clinician and patient care needs. Facilitate Veteran-therapist correspondence at time of re-evaluation as well as when treatment modification is required to be completed by evaluating therapist. The re-evaluation will be performed at a 30-day interval or if a medical status change is present. Prepares and presents written and oral reports for the attending physician, physiatrist, nursing staff, fellow therapists and all other professionals concerned with the Veterans care. May be required to administer Minimum Data Set. It is essential that the incumbent demonstrate exceptional verbal and written communication skills with peers and superiors, in order to speak in front of groups, and write policies, procedures, memoranda, and training materials. These skills will be used to facilitate communication between clinical needs and administrative needs, and for purposes of staff education. Program Development: The position is responsible for assisting in the development of Telerehabilitation Integration within the Community Based Outpatient Clinics as outlined in Functional Statement Overview and General Description. Assists in the development and disseminates educational and training opportunities for interdisciplinary professionals and settings, such as to advance the core tenants for Physical Therapy and/or Telerehabilitation. Meets the needs of customers while supporting VA missions. Consistently communicates and treats customers (Veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner. Provides the customer with consistent information according to established policies and procedures. Handles conflict and problems in dealing with the customer constructively and appropriately. Individuals must meet at least one of the requirements below: (1) Associate degree from an accredited community college, junior college, college or university in a physical therapy assistant education program accredited by the Commission on Accreditation in Physical Therapy Education; OR (2)[Graduate of foreign physical therapy assistant degree programs that meet the educational requirement and have successfully passed the National Physical Therapy Exam (NPTE) for PTAs. OR (3) Graduate of military physical therapy assistant programs that meet the educational requirement and have successfully passed the NPTE for PTAs. Licensure. Current, full, active and unrestricted license as a PTA in a State, Territory or Commonwealth (i.e. Puerto Rico) of the United States, or District of Columbia. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS9: (1) Experience. One year of experience equivalent to the next lower grade. In addition, the candidate must demonstrate one of the following: (a) A minimum of 2000 hours of clinical practice directly in the specialty area, This time must be within the past five years; or (b) Advanced Practice Proficiency (APP) in the specialty area recognized by the American Physical Therapy Association (APTA); or (c) An additional academic degree related to the health care mission. Demonstrated Knowledge, Skills, and Abilities. In addition to the basic requirements, the candidate must demonstrate the following KSAs: (a) Ability to mentor other staff in advanced clinical guidelines and best practices. (b) Ability to serve as a subject matter expert (SME) for healthcare providers in specialty practice areas regarding physical therapy care. (c) Ability to present] advanced knowledge in the therapeutic interventions and clinical practice guidelines with the healthcare team. (d) Ability to interpret and apply clinical practice guidelines. Assignment. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty) and variety and be performed by the incumbent at least 25% of the time. PTAs at this level apply advanced practice skills to treat complex patients. PTAs possess an advanced level of clinical competence and serve as a SME in specialty practice areas. They serve as a subject matter expert in a rehabilitation program such as, but not limited to, geriatrics, amputee, brain injury, cardiac rehabilitation, orthopedics, neurological, safe patient handing, or any other functional debilitative conditions. They possess advanced practice skills and experience with a wide range of patient complexity. They apply advanced methodologies to a vast array of functional impairments and patients. They independently recommend and apply evidence-based treatment approaches within the plan of care. Physical Requirements: The physical therapist may engage in the following activities/positions/tasks as defined: Lifting/carrying up to 45 pounds or higher intermittently, frequently walking, standing, and sitting for up to 8 hours/day. Occasional stooping, kneeling, overhead reaching, fine manipulation, pushing and/or pulling. Reaching above shoulder, use of fingers, both hands required, repeated bending, near vision correctable at 13" to 16" to Jaeger 1 to 4, Far vision correctable in one eye to 20/20 and to 20/40 in the other, hearing with aid allowed, clear speech, and working closely with others or working alone.
