Job Duties

Provides clinical and administrative oversight of Medical Home and Care Coordination Case Managers and has operational oversight of Medical Home implementation within assigned primary care practices. This includes delivery of disease and case management services, population analysis, primary care practice redesign, systems of care coordination, process improvement strategies, barrier analysis and data management. Monitors staff performance and coordinates ongoing staff development strategies. Ensures staff compliance with regulatory requirement.

The successful candidate must have at least five (5) years of RN work experience, have the ability to work autonomously and be self-directed. This role is Monday through Friday, Exempt (salaried) and may require some travel.

  • Manages nurse case managers in a dedicated region and ensures effective delivery of complex case management, resource utilization intervention, and disease management strategies.

  • Monitors performance of case managers using a variety of performance feedback reports related to enrollment, quality, and efficiency outcomes.

  • Meets with staff on regular basis to evaluate performance via direct observation and mentoring.

  • Responds to deficiencies with appropriate modifications in strategy.

  • Researches and resolves health information process issues.

  • Assists in evaluation and implementation of effective information management systems.

  • Allocates staff coverage to provide optimal utilization of staff resources.

  • Posts, interviews, and hires new staff.

  • Serves as a mentor and coach for case management staff.

  • Fosters positive work environment for staff.

  • Evaluates staff development needs and provides opportunities for ongoing professional development.

  • Develop and coordinatesorientation of new nursing staff and monitors progress according to department standards.

  • Conducts baseline medical home practice assessment in the practice site implementation process.

  • This includes assessment of current workflows, division of labor, barrier analysis and suggested strategies for practice change.

  • Oversees and provides education to practice sites to facilitate practice knowledge of medical home strategies, role and responsibility of team, medical home tools, etc.

  • Facilitates monthly medical home site meetings and implements changes as determined and in collaboration with the assigned site and case managers.

  • Leads in the ongoing process improvement strategies for medical home development as well as implementation of strategies designed to improve efficiency outcomes in conjunction with assigned primary care sites.

  • Monitors and analyzes utilization and quality metrics to recommend and implement change strategies with assigned sites to improve overall staff and practice efficiency of readmissions, pharmaceutical use, radiology use, ecetra.

  • Identifies opportunities with other systems of care to identify and eliminate barriers and implement improvement strategies designed to improve transitions of care.
