Job Description:

Social Worker LICSW

As a member of an interdisciplinary treatment team, the social worker will provide assessment, supportive counseling and care coordination responsibilities and service coordination for patients and families referred and/or meeting hospital established high risk screening criteria. Population groups may include adult, geriatric, pregnant and on an outpatient basis as assigned. Scope of care includes intervention as needed in outpatient departments, The department is operational five days a week. Performs all other duties as assigned.

Education/Experience: A candidate will have successfully completed a master’s degree in social work and preferably will have had at least one year of health related social work experience. Licensure is mandatory.

Certification/License: Current Massachusetts license in social work (LCSW or LICSW)
