Nuvance Health Breast Care Coordinator in Danbury, Connecticut

Summary:

Provides prompt and coordinated care to ensure the patients� needs are met by expediting the coordination of all testing and services in the department for all diagnostic testing and procedures. Works with patients and their physicians ensuring timely access for any needed testing or procedures as well as any anticipated future care.

Responsibilities:

  1. Scheduling criteria: A) Coordinates scheduling for all biopsy patients and updates patient demographic information in the Soarian scheduling system in real time when patient is in the office, and via telephone with patients and physician's offices B) Coordinates scheduling all patients for 3,6,9, and 12 month follow up imaging as recommended by Radiologist in the Soarian scheduling system when patient is in the office C) Acquires the required prescription for all exams performed at the Danbury Breast Center D) Provides educational material pertaining to the specific exam/procedure ordered to the patient E) Emails daily updates on the biopsy schedule to appropriate staff F) Answers the telephone promptly and courteously. Takes detailed messages to ensure accurate and timely responses. Retrieves messages and responds to them in a timely fashion

  2. Breast Biopsy Screening/Preparation Criteria: A) Works with physician's office to obtain correct prescription for follow up imaging/procedure B) Provides Radiologist with pertinent information to review for upcoming procedure C) Provides patient or office instructions in preparation of upcoming procedure

  3. Biopsy Documentation Criteria: A) Ensures all biopsy documentation and forms are prepared for day of exam B)Ensures all required paperwork is complete and accurate for Radiologist to review prior to exam date

  4. Records Criteria: A) Assists in obtaining all necessary imaging prior to the patient arriving for their exam B) Facilitates copies of images and reports for patients who need to see a breast surgeon as needed C) Ensures all required paperwork for the following day�s procedures are complete and accurate

  5. Patient letters/Tracking Criteria: A) Responsible for running all daily BI-RAD zero reports and scheduling patients for follow up imaging to include mammography, ultrasound, and MRI for the Danbury Breast Center, RDI, NMH B) Responsible for sorting and mailing out all BRAD 1 and 2 result letters, matching self-referred letters as applicable for the Danbury Breast Center, RDI, NMH C) Responsible for sorting and mailing out all patient reminder letters for the Danbury Breast Center, RDI, NMH

  6. Miscellaneous Criteria:Attends required meetings with the clinical management team and brings important topics to discuss

  7. Performs other duties as assigned

  8. Fulfills all compliance responsibilities related to the position

Other Information:

Education:HS graduate or equivalent

Skills: Required: Data entry, customer service and organizational skills.

Minimum Experience: 3 years

Desired: familiar with Cerner, Document Imaging, PACS, Soarian Scheduling

Desired: Exhibits a warm, caring, and professional manner. Maintains a positive attitude and demonstrates respect and courtesy. Excellent verbal and written communication skills. Ability to work independently. Understanding of medical terminology
