PRN Service Coordinator
Texas Health Dallas is looking for a PRN Service Coordinator
Work location: Texas Health Dallas, 8200 Walnut Hill Ln, Dallas, TX 75231
Work hours: PRN 730a-8p, days will vary.
Here’s What You Need
· H.S. Diploma or Equivalent required.
· 1 year of related experience required.
What You Will Do
Manage the delivery of services provided by Texas Health's Service Provider Network (SPS) partners to ensure seamless delivery of care to Acute Care at Home patients
Coordinate and schedule home visits, equipment and pharmaceutical delivery, and lab tests with community partners and vendors from order to service fulfillment
Manage complex and multidisciplinary real-time scheduling needs of a diverse patient population
Diligently follow-up on assigned tasks and communicate updates to the team in an efficient and reliable manner
Collect and organize patient data
Maintain records and databases including record set-up, uploading documents and maintenance
Maintain supplies and documentation, as needed, to support the unique ACH care delivery model
Collaborate with leadership to develop and implement new processes and systems, with potential to train and orient new staff
Perform other administrative support duties as required
Verify technology works correctly in platform
Manage/Oversee patients flow and reassign
Register patient in EPIC
Communicate with patient over secure chat as needed
Independent, proactive problem-solving skills
Excellent organizational skills with attention to detail
High level of service deliverability including multi-tasking, ability to prioritize work and meet deadlines and adapt to a dynamic environment
Strong written and verbal communication skills
Computer literacy, including command of Microsoft word, PowerPoint, and Excel with ability to learn new software as needed
Ability to demonstrate respect and professionalism for patients’ rights and individual needs, including demonstrated ability to maintain HIPAA compliance and other confidentiality
Ability to work independently and as a team player
Analytical skills and ability to resolve technical problems if needed
Excellent customer service skills, including the ability to manage and triage complex telephone calls
Comfortable working with a diverse patient population and multidisciplinary team, including nursing, physicians, and ancillary service providers such as PT, OT, skilled nursing, etc.
Ability to maintain a positive attitude and uphold the ACH culture
