Job Summary:

As a Behavioral Health Case Manager, you'll coordinate care, provide direct services, and assist clients in accessing a range of community resources to support their individualized treatment plan goals. Key Responsibilities:
Provide all services within the organization's Code of Conduct and policies
Demonstrate understanding of health service systems and assist clients in accessing resources
Coordinate care and participate in the development of the client's individualized treatment plan
Consult and cooperate with community resources to facilitate linkage, referral, and follow-up
Provide direct services to adults, children, and families; coordinate family involvement
Help clients navigate financial entitlements and access healthcare coverage
Assist clients in learning skills for personal health management and self-advocacy
Monitor and assess client functioning and progress; update treatment team
Demonstrate understanding of mental illness and its treatment
Evaluate services to identify necessary improvements and increase outcomes
Provide continuity of contact and coordinate access to housing, social services, and other resources
Understand and respond to client rights; advocate on their behalf
Assist clients in developing appropriate social skills and behaviors
Serve as a link between clients and community services/resources
Perform other duties as assigned
Education and/or Experience Qualifications:
Bachelor's degree in social service, education, behavioral health, or a related field
Documented experience in case management services preferred
Certified as a Behavioral Health Case Manager II with the Oklahoma Department of Mental Health and Substance Abuse Services
Required License/Certification (one of the following):
- Current certification as a Behavioral Health Case Manager II
Additional Qualifications:
Knowledge of case management and rehabilitation principles related to mental/behavioral health
Understanding of the challenges faced by individuals with mental illness and available community resources
Knowledge of treatment modalities and legal requirements in mental/behavioral health
Ability to communicate ideas clearly and effectively, both verbally and in writing
