Qualifications
:
- Bachelor's degree from accredited school of Social Work or related field is required
- Must possess any certifications/licensures as required by State of employment to practice in long-term care
Administrative Functions:
Plan, develop, organize, implement, evaluate, and direct social services programs of facility.
Develop, administer, and coordinate department policies and procedures
Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to Administrator
Participate in community planning related to interests of facility and services and needs of resident and family
Perform administrative requirements, such as completing necessary forms,reports etc. and submitting such as required
Make written and oral reports/recommendations to Administrator and relevant department heads concerning operation of social service department
Be prepared to handle emergencies as they arise (i.e. rescheduling work assignments and work)
Specific Functions:
Orients resident and family to Nursing center, helping both adjust to new environment
Develop a social history on each resident
Develop social assessment and care plan; which identifies medically related social and emotional problems and needs with realistic goals and specific actions to be taken
Answer inquiries of admission and provide tours to prospective family members/residents
Be actively involved in pre-admission and admissions processes
Assist in continual updating of admissions waiting list
Interviews resident/family as necessary and in private setting
Ability to listen and act compassionately with residents/family members; must be able to work on own and maintain related jobs
