POSITION SUMMARY

  • The Licensed Practical Nurse (LPN) in Home Care and Hospice plays a vital role in providing high-quality, compassionate care to patients in their homes. The LPN is responsible for providing direct patient care to the home health and hospice patients. Direct patient care provided by the LPN would include implementing nursing care plan initiated by a Registered Nurse, physical assessment, monitoring vital signs, providing support and education to patients with chronic conditions or post-surgical needs, wound care, venipuncture, urinary catheterization. This role requires strong clinical skills, empathy, and the ability to work independently in a home setting while collaborating with the broader healthcare team.

    POSITION QUALIFICATIONS

  • Education and Experience:

    • Graduate of an accredited school of nursing.
  • Previous experience in home health or hospice, clinic, rehabilitation setting.

  • License/Certificates:

  • LPN License to practice in the State of Minnesota. American Heart Association BLS certification within 30 days of hire. Valid MN Driver's License required.

    ESSENTIAL RESPONSIBILITIES

  • Patient Care:

  • Deliver direct nursing care in accordance with individualized care plans.

  • Physical assessment and electronic medical record documentation at point of care.

  • Assist patients with daily living activities such as bathing, dressing, and grooming when the agency requires additional support to meet the needs of patients.

  • Administer medications and perform treatments as prescribed.

    Clinical Support:

  • Report changes in patients' conditions to the supervising RN or physician.

  • Collaborate with multidisciplinary teams to ensure comprehensive care.

  • Assist in implementing patient care plans designed by the supervising RN.

  • Provide wound care, catheter care, and ostomy care as needed.

    Patient & Family Education:

  • Educate patients and their families about care plans, promote health and wellness, educate patients and families disease management, and safety in the home environment.

    Documentation:

  • Maintain accurate and timely records of patient care, treatment, and progress using the appropriate electronic health record system.

  • Ensure all documentation complies with regulatory and agency standards.

    Compliance & Safety:

  • Follow infection control protocols and safety guidelines.

  • Adhere to local, state, and federal regulations related to nursing care in the home setting.

  • Demonstrate Standards of Excellence when other duties are assigned.
