Amergis School-based LVN. Trach/Vent in Covina, California

Amergis is hiring School-based LVN's to provide trachesotomy care for students!

Schedule: Full-time

Pay: $31-35/hr DOE

Location: Pomona, Claremont, Covina, Charter Oak, Walnut, and surrounding area

The LPN/LVN Tracheostomy School Nurse assumes responsibilityand accountability for the application of the nursing process and the deliveryof student/client care and/or contracted services in a school setting. The LPN/LVN Tracheostomy School Nurseprovides services in accordance with the school district and Amergis' philosophy, standards, policies and physician orders.

Essential Duties and Responsibilities:

  • Utilizes the nursing process to plan, implement and evaluatestudent’s care needs

  • Demonstrates on each assignment the provision of skillednursing care in accordance with prescribed orders/IHP

  • Performs interventions, treatments, and medicationsadministration according to the identified priorities as outlined in theIHP

  • Monitors and documents effectiveness of interventions,treatments, and medications

  • Completes documentation per client expectations

  • Coordinates and facilitates services as requested betweenthe school, local health agencies, and other community resources to meet theneeds of the student

  • Communicates with the school district personnel aboutstudent’s plan of care/IHP

  • Notifies District School Nurse/Amergis Supervisor/Parent/Physician as outlined in the IHP regarding the following: changes in condition of student and Physiciannotification to obtain necessary orders

  • Collaborates with school/education personnel/other healthagencies as requested, to design and meet student’s health care needs

  • Performs emergency trach change if needed/required bystudent’s respiratory status

  • Provides trach care to students to maintain the patency ofthe tube while minimizing risk for infection

  • Performs trach suctioning and cleaning per providerorders

  • Evaluates and assures student safety at all times

  • Adheres to Amergis and school districtpolicies

  • Maintains accuracy in student health record

  • At request of school district participates in student careconferences/Individualized Education Program (IEP)

  • LPN/LVNs should direct questions regarding orders andchanges in condition to a Supervising RN (DN and/or Clinical Designee)

Minimum Requirements:

  • Licensed as a Licensed Practical/Licensed Vocational Nursein state(s) where practicing

  • One year of School Nursing experience, preferred

  • At least one year of recent tracheostomy experience isrequired, unless approved by Amergis Clinical Designee

  • Completes tracheostomy competencies successfully

  • Complies with all relevant professional standards ofpractice

  • Participation and completion of Amergis' Competency programwhen applicable

  • Current CPR if applicable

  • TB Questionnaire, PPD or chest x-ray if applicable

  • Current Health certificate (per contract or stateregulation)

  • Must meet all federal, state and local requirements

  • Successful completion of new hire training as applicable tojob site

  • Understand patient confidentiality and HIPAArequirements

  • Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required

  • Computer proficiency required

  • Must be at least 18 years of age
