Guthrie Discharge Planning Assistant - Psych Services - Part Time in Cortland, New York
Communicates discharge recommendations of the clinical team to patient, care givers and/or interdisciplinary team. Responsibilities: Participates in care planning and communicates information between patient and team. Documents plan in medical record.
| Gathers and arranges the necessary materials to enable patient discharge.
| Orders supplies/equipment for patient specific needs as identified by clinical team.
| Performs administrative and clerical duties in support of unit/department.
The pay for this position ranges from $16.00-$22.37 per hour.
