The State of New Hampshire, Department of Health and Human Services, Division of Public Health Services has a full time vacancy for State Health Improvement Manager.

SUMMARY:

Coordinates the development and implementation of State Health Assessment, State Health Improvement Plans, and Community Health Improvement Plans, and Performance Improvement functions for the Division of Public Health Services within the Bureau of Public Health Systems, Policy and Performance. Recommends alignment of resources to help support community and state improvement plans. Formulates, refines, implements and evaluates performance management and related quality improvement systems at the state and community level in order to increase overall organizational effectiveness and efficiency goals, to prepare for national Public Health accreditation and to assure the organizations core public health functions are being delivered in a manner that will positively impact the publics health.

RESPONSIBILITIES:

Manages development of the State Health Assessment, State Health Improvement Plan and the coordination of these with Community Health Improvement plans developed by Regional Public Health Networks.

Coordinates with DPHS programs and other stakeholders to provide training and technical assistance to the Regional Public Health Networks related to Community Health Improvement Plans.

Evaluates agency performance regarding progress in meeting public health goals and objectives; makes preliminary recommendations for the improvement of program performance in meeting agency goals and objectives; assist in sustaining a culture of quality improvement. Coordinates and facilitates communication with local health agencies and contracted public health service providers on performance and quality initiatives to ensure alignment with DPHS goals and objectives, and to ensure optimal use of resources.

Analyzes the efficiency and effectiveness of DPHS policies, practices and programs; evaluates and improves the effectiveness of program practices, partnerships, use of resources, and the impact the systems improvements have on the publics health; makes recommendations based upon evidence, data, and financial analysis to modify operational procedures for increased efficiency and effectiveness.

Coordinates training needs for program practices, and performance management and quality improvement processes for DPHS staff.

Supports the development of work needed to apply to national Public Health Accreditation Board to become an accredited state public health agency. Develops systems to assess needed documentation for accreditation by working with DPHS Management team and respective staff to review standards and documentation, identify gaps to be addressed prior to seeking accreditation, and recommend development of documents and processes needed to close such gaps. Supervises staff responsible for accreditation documentation and coordination.

Prepares and reviews grants and contracts to support the program, including progress reports to funding agencies, including the Preventive Health and Health Services (PHHS) Block Grant Program. Recommends alignment of resources to support community and state health improvement.

As the Performance Improvement Manager (PIM), participates in a national network of public health improvement professionals, sharing best practices and tools to bui d capacity through peer exchanges, and to ensure public health sciences are applied in the delivery of the 10 Essential Public Health Services.
