Responsibilities Initiates telephone contact and completes a health risk assessment. Escalates complex or urgent needs to a supervisor. Connects patients to the right program once needs are identified. Develops individualized self-management skills to patients and families through education and resource provision. Provides organization of electronic communication related to survey results of the health risk assessment. Documents within the electronic medical record. Participates in orientation and continuing education of staff and students as appropriate. Qualifications Education: High School diploma, with background in health care, public health, or related clinical field a plus. Skills: Excellent customer service skills with the ability to communicate effectively through multiple mechanisms including phone and writing. Experience: Experience working in healthcare in a patient facing role, preferred.
