We are hiring for a Part-Time Practice Performance Advisor to join our Evolent Care Partners Performance Team.

The Part-Time Practice Performance Advisor will serve as the local expert and resource for Evolent Care Partner’s national network. This role will work in collaboration with other subject matter experts to drive the overall maturation of the assigned region for value-based care initiatives.

Collaboration Opportunities:

The Part-Time Practice Performance Advisor will travel throughout designated territory to build relationships with physician practices to support physician engagement, clinical practice transformation and ensure Triple Aim achievement by conducting the key functions below.

What You Will Be Doing:

• Playing a critical role in performance management by providing insight from data and reports; developing and implementing action plans for improvement, conducting rapid cycle process improvement and measuring progress and outcomes

• Integrating key ACO priorities including care management, utilization management, risk adjustment, annual wellness visit, quality and medical expense and cost savings initiatives into practice and ensuring goals are met by assigned practices Serving as the primary local operations liaison to practice managers and staff for assigned practices for Evolent Care Partner’s central operations team as needed, providing training and education, assisting with local communications efforts, meetings, and event coordination, etc.

• Leading the transition of innovative ideas from pilot to implementation and sharing best practices across all practices and at local ACO regional forums

• Coordinating with regional Medical Directors, network physician champions and performance and operations teams to develop strategy and content for local ACO forums.

• Facilitating practice manager and provider meetings as well as local ACO forums in the assigned territory

• Collaborating with ACO team members, subject matter experts, and local medical directors/physician champions, ensure progress towards ACO, local network, practice, and provider improvement goals

• Building strong relationships by working with go-to physician leaders and leveraging practice relationships to engage, implement, and sustain ACO priorities

• Building relationships with and serve as liaison to community resources and/or ancillary providers, skilled nursing facilities, etc. as needed

Qualifications (Required and Preferred):

• Must reside in the state of Hawaii, preferably on the island of Oahu

• Must have a valid Driver's License and ability to travel locally/regionally up to 75% of the work week

• Bachelor’s Degree or equivalent work experience

• 5+ years of leadership experience in practice management, provider relations and project management

• Proficiency in Microsoft Office products (Outlook, Word, Excel and PowerPoint)

• 3 years+ experience in practice re-design work including Patient Centered Medical Home, Advanced APMs, Practice Transformation, Quality Improvement, ACO development, Ambulatory Care, Quality and Efficiency metrics, and EMR

• IPA and/or ACO experience with a provider perspective and strong orientation to value-based care principles and the health policy landscape

• Working knowledge of clinical quality metrics such as RAF, NCQA or National Quality Forum type metrics

• Experience in data analytics comprehension

• Excellent written and oral presentation skills, with the ability to engage, inspire, build credibility and trust

• Highly organized and self-motivated individual with ability to adapt to various workspaces and work autonomously

• Collaborative working style with the ability to work across different teams, areas of expertise, and adapt to ambiguous environments and clientele

• Hypothesis driven to identify trends, predict issues, highlight critical areas, and develop corrective action plans

• Demonstrated track record of change management and an ability to identify and share best practices across multiple stakeholders

Technical Requirements:

We require that all employees have the following technical capability at their home: High speed internet over 10 Mbps and, specifically for all call center employees, the ability to plug in directly to the home internet router. These at-home technical requirements are subject to change with any scheduled re-opening of our office locations.

The expected base salary/wage range for this position is $41.00 per hour. This position is also eligible for a bonus component that would be dependent on pre-defined performance factors. As part of our total compensation package, Evolent is proud to offer comprehensive benefits (including health insurance benefits) to qualifying employees. All compensation determinations are based on the skills and experience required for the position and commensurate with experience of selected individuals, which may vary above and below the stated amounts.
