We're Hiring! High Fidelity Wraparound (HFW) Facilitator - Remote
Pikes Peak Respite Services is looking for a dedicated and compassionate High Fidelity Wraparound (HFW) Facilitator to join our team. This remote position offers an opportunity to work with youth facing behavioral health challenges and support their families in fostering long-term, community-based solutions.
About the Role: The HFW Facilitator will lead efforts to coordinate care for youth with complex needs by collaborating with multidisciplinary teams to develop and implement individualized care plans. This role empowers families, promotes community integration, and reduces reliance on formal services through sustained family-driven care.
Key Responsibilities:
Facilitate coordination among families, service providers, and community supports. Conduct assessments to identify youth and family needs. Develop and implement individualized, culturally responsive care plans. Support families in building skills to coordinate their own services. Monitor and evaluate the success of care plans, adjusting as needed. Provide crisis stabilization and ensure the safety of youth and families.
Qualifications:
Experience working with youth and families in mental health, child welfare, education, or juvenile justice settings. Bachelor's degree in social work, psychology, or related field preferred (or equivalent lived experience). Strong cultural competency and ability to engage diverse communities. Excellent collaboration and communication skills. Wraparound Facilitator experience preferred.
