We are currently hiring a Landlord Liaison / SOAR Case Manager, to work in our Schoharie, Delaware and Otsego County Offices. As the Landlord Liaison / SOAR Case Manager you would be responsible for:
Participate in the Coordinated Entry meetings to keep current in client information, housing needs, and client needs for SOAR
Seek and identify available housing units and collaborate with landlords and case managers to support client housing placement and retention
Collect all medical documents regarding individuals applying for SSI/D. Complete SOAR applications for individuals who have a physical and/or mental disability. Document the application. Conduct regular follow-up, barrier removal and problem solving until all applications/processes/appeals have come to final resolution
Participate in Catholic Charities meetings and functions, including: Staff meetings, Christmas Board/Staff event and other staff training or appreciation days, Annual Dinner.
Landlord Liaison / SOAR Case Manager Minimum Salary: $28.57 / Per Hour
Landlord Liaison / SOAR Case Manager Maximum Salary: $28.57 / Per Hour
Landlord Liaison / SOAR Case Manager Schedule: Monday – Friday Day shift
Qualifications:
Associates Degree in Human Services, Law Enforcement or related field.
Valid Driver’s license for travel within counties assigned.
