Abraxas Academy Case Manager in Coal Township, Pennsylvania

CHOR YFS *is looking for a *Case Manager at our Coal Township Program. In this role, you will maintain all resident medical and individual service plan portions of records and ensure compliance with standards set forth by all licensing and regulatory bodies (DPW, Managed Care Organizations, & COA). You will also, coordinate all health care appointments and ISP meetings to ensure compliance and standards set forth by all licensing bodies. Facilitates in-home visits and tracks all visitations. Coal Township Program is a 16 bed Psychiatric Residential Treatment Facility.

     *Duties and Responsibilities: * 

  • Maintains all client records and medications utilizing the Electronic Health Record system (Credible). 

  • Ensures emergency medical and emergency contact information is up to date on the Electronic Health Record for all clients. 
  • Reviews and approves all medical incident reports. 
  • Schedules and tracks all routine, specialist, and follow-up appointments, ensuring compliance with medical regulations set forth by all licensing and regulatory bodies. 
  • Secures private and/or medical assistance health insurance information, if not supplied at time of intake. 
  • Orders, maintains, and monitors prescription medication, ensuring required doses are available for clients as prescribed. 
  • Oversees administration of medication, training necessary staff in mandated protocols.  
  • Notifies parents and/or legal guardians of all client medical appointments and changes in client medications. 
  • Keeps and maintains the ISP portion of the resident's record ensuring compliance with regulations set forth by all licensing bodies, referring counties, and Managed Care organizations. 
  • Notifies all involved parties of scheduled ISP meetings, 30 days in advance of the meeting.  Documents such notifications in EHR. 
  • Completes behavior adjustment portion (goals and summary) of treatment plans, integrating goals identified and submitted by Primary CCWs. 
  • Mentors direct care staff with regard to creating and adjusting behavioral goals. 
  • Updates daily sheets, ensuring accurate behavioral goals are included 
  • Attends and help facilitates all ISP meetings. 
  • Coordinates and attends meetings with counties and stakeholders  
  • Oversees the orientation and ongoing training of all direct care staff. 
  • Obtains and maintains certification as a MAB (Management of Aggressive Behavior) trainer 
  • Coordinates Motivational Interviewing (MI) training for all residential staff.  Includes the provision and scheduling of MI 101 training for new hires, booster trainings, and coding/interviewing. Schedules training for coaches. 
  • Provides CPR and 1st Aid training to all employees as required.  Maintains records of timelines associated for all employees in order to ensure compliance. 
  • Obtains and maintains certification as a Medication Administration trainer 
  • Assists in medical appointment transportation, when necessary.

Education/Experience 

Bachelor's degree with a background in the creation and implementation of behavioral modification goals.  

Language Ability 

Ability to read, write, and comprehend information in English.  Ability to effectively communicate with peers, clients, employees, and administration.  Abili y to professionally write detailed reports and correspondence.
