Divinity Home Care Home Health Aide in Clinton Township, Michigan
We are looking for licensed CNAs to join our team at Divinity Home Care as a Home Health Aide. We are a 4-star Medicare A Home Care Agency, providing services throughout Macomb County.
Position starts contingent. Hours are flexible, and you have the ability to arrange visits around your schedule!
Pay starts at $20 per visit plus mileage, which is reimbursed for required travel outside of your first patient and home from your last patient.
Responsibilities:
- Work with intake and case managers to be given a list of patients to see each week.
- Reach out to the patients to schedule visits to meet frequency.
- Assist with showers and adls for home-bound patients.
- Work with other care team members (RN, PT, OT, SLP) to instruct patients on how to perform tasks more independently before being d/c from home care.
- Providing personal care including: bathing, oral hygiene, skincare, nail care, changing bed linens, assisting with dressing/undressing, and assisting with toileting.
- Keeping patient's living area clean and orderly, as appropriate
- Reading and recording temperature, pulse, respiration, and blood pressure when ordered (with appropriate completed/demonstrated skills competency).
- Assisting in ambulation and exercise according to the plan of care.
- Assists the therapy personnel as needed with the rehabilitative process. Understands the normal range of motion and positioning.
- Meeting safety needs of patients and using equipment safely and properly (bed rails, bedside commodes, walkers, wheelchairs, etc.)
- Encourages the patient to become as independent as possible according to the Home Health Aide care plan.
- Respects patient rights, privacy, and property.
