Position Purpose
CHHA provides personal care and promotes maintenance and rehabilitation if applicable of the patient under the supervision of a Registered Nurse and in accordance with a plan of treatment. CHHA facilitates a safe home environment. Must have knowledge and skills necessary to provide patient care that is age specific to the patients served. Ages served range from infant to adult.
What You Will Do
Provides overall personal care to patients following the written plan of care. Allows for patient input and preference in this plan.
Assists patients with or provides patients with personal care as directed. Ensuring safety of patient.
Assists patients in ambulating as directed by nurse. Performs simple procedures as an extension of therapeutic services including P.R.O.M. and A.R.O.M.
Transfers patients from bed to chair/wheelchair and back using proper lifting and transferring techniques and/or equipment. (Gait belt, Hoyer lift, bench, walker, cane, wheelchair.)
Provides simple meal preparation, linen change, personal laundry, tidying bedroom, tidying bathroom, dishwashing, and light housekeeping as directed.
Minimum Qualifications
Current Certified Home Health Aide – Iowa Registry required.
Expected to take weekend calls, holiday calls, and make home visits as required.
Must apply and obtain Illinois licensure.
Must provide current, valid driver's license.
Must provide current, valid auto insurance.
One-year health aide experience required; Home Care/Hospice experience preferred.
High school graduate or equivalent.
Must be BLS certified within 3 months of hire.
Must possess good oral, written, communication, and observational skills.
Must be able to complete work efficiently, effectively and independently.
Hospice training if applicable.
Must be comfortable operating in a collaborative, shared leadership environment.
Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission and vision, goals, and values of Trinity Health.
