Cleveland Clinic Social Worker - Endocrinology in Cleveland, Ohio

Cleveland Clinic is an internationally recognized leader dedicated to medical and surgical conditions of the endocrine systems, including cancer, diabetes, and obesity. As a Social Worker in the Department of Endocrinology, you will assist patients and their families in achieving improved health outcomes by reducing obstacles that interfere with their ability to use health resources effectively. This team works collaboratively to optimize service for patients, support them, and connect them with community resources.

A caregiver in this position works days, 8:00am-5:00pm.

A caregiver who excels in this role will:

  • Assess patient and family's psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness and ability to cope.

  • Assess patient and family's financial, social, emotional, educational, and environmental resources and identifies needs based on patient's current medical condition.

  • Develop an individualized plan of care based on the findings of the assessment and needs of the patient and family/caregiver as related to the patient's medical condition.

  • Participate in Discharge planning activities for complex patients to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers.

Minimum qualifications for the ideal future caregiver include:

  • Licensed Social Worker (LSW) in the current State.

  • Proof of Basic Life Support (BLS) through American Heart Association (AHA) must be presented upon hire.

  • Excellent communication and negotiation skills necessary to work collaboratively with multidisciplinary teams for optimal outcomes.

  • Show ability to work independently and with teams.

  • Must be flexible, open-minded, and adaptable to change.

Preferred qualifications for the ideal future caregiver include:

  • Three years of experience in healthcare systems to include exposure to and experience in pre-acute care.

  • Social Worker OR Case Management Certification (CCM) (within one year of eligibility).

  • Hospital Social Work experience.

  • Knowledge of community resources.

  • Dual language English/Spanish.

Physical Requirements:

  • Requires normal or corrected hearing and vision to normal range.

  • Ability to perform work in a stationary position for extended periods.

  • Ability to communicate and exchange accurate information

  • Ability to work with physical records, such as retrieving and filing. Ability to operate a computer and other office equipment.

  • Ability to travel throughout the hospital system.

  • Requires normal or corrected hearing and vision to normal range.

  • In some locations ability to lift up to 10 pounds
