Ohio Means Jobs Cleveland Cuyahoga County is sourcing for a Crisis Center Family Service Coordinator for a newly build center on the East side of Cleveland. Clinical/Family-Based Services ? Screen inquiry calls, validate referrals, manage the Crisis and Pediatric Crisis Nursery waiting list, and complete all qualified intakes. Documents all contacts to the waiting list, including referrals made. Reports any allegations of abuse or neglect to the Cuyahoga County Division of Children and Family Services. ? Manage Respite and Tier 1 admitted families and children including all case management activities and support including, but not limited to, making referrals and coordinating services. Assists with overflow cases as needed from Tier 2 and Tier 3 families. As appropriate utilize KIPS observation method, and the Conscious Discipline and Nurturing Parenting Curricula as well as the Developmental Profile-3. ? With assigned families develop initial service plan and all supporting goals. Lead discharge planning including making referrals for service post discharge, communicating with service providers and any activities that strengthen family stability. ? Attend staffings and meetings with referring agencies and Cuyahoga County Division of Children and Family Services as assigned. ? Create and implement a family education plan for assigned families including classroom and hands-on experiences. ? Manages assigned clients' medication and school needs. ? Work directly with families to determine frequency and scheduling of visitations. ? Observe and provide direct support to caregivers to ensure successful and meaningful visitation that allows for the practice and implementation of skills learned in parent education to support the parent's educational plan. ? Assist with childcare duties as needed. ? Be a part of the on-call rotation in addition to regularly scheduled work hours with one additional weekend day worked per month. ? Assist in scheduling and completing outreach appointments to community partners and referring agencies as assigned by the Director of Programming and Clinical Operations. Staff Management ? Models and oversees the appropriate implementation of behavior management techniques by Childcare Providers. ? Report any performance concerns to the Nursery Operations Manager and HR Manager Administrative Tasks ? Documents all contacts with clients and partnering agencies (phone-based, in person meetings, team meetings, referrals, etc.). ? Assists Director of Programming and Clinical Operations in tracking and reporting client service units and outcomes. ? Attends program team meetings, administrative meetings, and outreach meetings as assigned and complete any assigned projects ? Undertake other assignments and projects as requested by the Director of Programming and Clinical Operation, or other staff acting as a Project Manager. ? Provide Quality Assurance on all children's residential files and reports deficiencies to Director of Programming and Clinical Operations and Compliance and Quality Coordinator. ? Know, understand and apply rules pertaining to childcare as outlined in 5101:2-5 and 5101:2-9, and all applicable CARF, Medicaid and Ohio MHAS standards ? Attend and assist with the organization and execution of special events including Annual Luncheon, Deck the House, and other speaking engagements or fundraising events as assigned. ? Must have a Bachelor's Degree in Social Work at minimum. ? Must be a Licensed Social Worker (LSW) in the state of Ohio ? 21 years of age minimum and a valid State of Ohio driver's license ?
