OPS Advanced Practice RN Specialist
Your Specific Responsibilities:
This position is an Advanced Practice Registered Nurse who will provide health care services to the unsheltered homeless as part of the Health Care for the Homeless Program. This position will provide PRN coverage for the Bayside Clinic, Mobile Medical Unit, and Street Medicine team during vacations, sick leave, FMLA, or other mandatory training events. It may also require the individual to work irregular shifts on Monday-Thursday from 8am-8pm, Friday from 8am-5pm, and Saturday from 8am-12pm depending on need. Duties of the position include:
Provides comprehensive health care to homeless clients via outreach, in clinic setting, and/or via telehealth.
Evaluates and performs complete physical examinations.
Analyzes and interprets patient histories, symptoms, physical findings, and/or diagnostic information to develop appropriate diagnoses.
Diagnoses and treats acute health care problems such as illnesses, infections, or injuries.
Provides wound care.
Orders, conducts, interprets, and/or evaluates diagnostic tests to identify and assess patient's condition.
Orders, reviews and interprets laboratory tests to identify and assess patient's condition.
Prescribes and administers medications based on patient characteristics, efficacy, safety, and cost as legally authorized.
Makes medical diagnosis and prescribes treatment within scope of service.
Develops treatment plans based on scientific rationale, standards of care, and professional practice guidelines.
Makes appropriate follow ups and referrals.
Educates patients about self-management of acute and chronic illnesses, tailoring instructions to patients' individual circumstances.
Provides consultation and technical assistance in evaluating the appropriateness of client care as it relates to primary care.
Documents all services in EHR timely and accurately.
Communicates with medical team and other colleagues.
Complies with all UDS measures, clinical standards and selected goals for quality of care.
Provides excellent customer service.
Completes case management, including record review and lab review.
Keeps supervisor and Medical Director informed.
Completes timesheet, EARS, and mandatory training in a timely and accurate manner.
Completes training to promote professional growth.
Participates in peer review and provider, team, and other meetings as assigned
Performs other related duties as assigned including emergency duty.
Required Knowledge, Skills, and Abilities:
Knowledge of nursing principals, practices and techniques.
Knowledge of human anatomy and physiology.
Ability to administer prescribed medications.
Ability to perform complete physical assessments of patients.
Ability to identify and evaluate illnesses.
Ability to initiate diagnostic laboratory testing and evaluate the results.
Ability to manage the care of patients.
Ability to compile and evaluate medical histories and other clinical and laboratory data.
Ability to plan, organize and coordinate work assignments.
Ability to use independent judgment and decision-making.
Ability to work independently and manage one’s own time.
Ability to use critical thinking.
Knowledge of customer service principles.
Ability to communicate effectively both verbally and in writing.
Ability to develop and maintain effective working relationships.
Skill in computer software programs used to facilitate job duties.
Qualifications:
Minimum-
A clear and active license in the state of Florida to practice as an Advanced Registered Nurse Practitioner
Active DEA licensure in the state of Florida
A valid Florida driver's license
Access to reliable personal transportation
Masters Degree
Preferred-
Five or more years of experience utilizing an electronic health record in a clinic setting
Five or more years of experience working as an ARNP in an internal medicine or family practice clinic setting
Experience working as a nurse in an outreach setting
Experience working at a Federally Qualified Health Center
Experience providing clinical care to patients experiencing homelessness
Experience providing clinical care to clients via Telehealth
