The Direct Support Professional works with a  team to ensure people's progress toward self-reliance, personal health, safety, and community connections through specific therapeutic plans and goal setting. They are the main support for the people we serve.  The Direct Support Professional is essential in assisting the person to have a positive environment and fulfilling life.

 Essential Functions / Responsibilities:

  • Staff are required to be aware of and take responsible for the individual's health and safety (shelter, food, health), ADL (activities of daily living) and IADL (instrumental activities of daily living). 

  • Coordinate scheduled times to be worked with the person with input from the program manager.

  • Must work scheduled shifts in their entirety and maintain timely attendance.

  • Become familiar with the person's individual support plan and programs including behavior supports, medical supports, financial support etc.  Understand that each person's needs are different and diligently seek ways to individualize each person's supports.

  • Actively implement and document the supports to ensure the person's needs and goals are being met.

  • Assist the person as needed with household management tasks (cooking, cleaning, bill paying etc.)  Provide a positive attitude while modeling the basic procedures to ensure the person's environment is clean, meeting health and safety standards.

  • Assist people to gain access to the community and see that opportunities are provided for developing relationships/social skills.  This includes providing transportation for shopping, work/volunteer activities and leisure activities.

  • Ensure Human Rights Restrictions are being reviewed regularly and progress and roadblocks towards removal of these restrictions is being documented.  Assist the person to continue to work towards removal of all human rights restrictions.

  • Schedule medical appointments, and educate the person you serve on important issues.  Know the difference between urgent and regular medical care.  Know the medications and side effects for the person you support.  Understand the impact of medical problems such as seizures, diabetes, etc.  

  • Ensure the accurate maintenance of financial records for the person you support. Keep property inventories current and available.

  • Document progress, successes and any incidents daily.  Complete therapeutic program notes to ensure they maintain a high level of quality.  

  • Assist program leaders with written summaries to support coordinators.  Keep essential medical emergency and personal information (such as changes in address, phone, parent phone, primary physician, etc.) current and available.  

  • Complete GER accurately and ensure they are complete and submitted in a timely manner.  

  • Be an active advocate for the person's rights

  • Attend all required staff training (CPR, SOAR, monthly staff training, etc...).

  • When taking time off arrange with someone to cover vacation and when possible, sick days. Help cross-train backup staff.

  • Work with other staff in a cooperative way to cover vacations, weekends, and emergencies so all of our people have their needs met at all times.

Requirements:

  • Must be 18 years or older.

  • High school diploma or GED preferred. 

  • Must be able to work a flexible schedule to meet the needs of the people we serve.

  • Must have personal vehicle or reliable transportation. 

  • Must be able to lift up to 20 lbs. May be more lifting if the needs of the person require.

  • Must have basic understanding of record keeping and basic math skills.

  • Positive, proactive attitude.  Strong interpersonal and organizational skills, including compassion, ability to maintain control of difficult situation, have self-confidence, honesty, resilience, integrity, ccountability, ability to follow-up and be compatible with our people

  • Must have strong work ethic, good writing skills, and patience to fill out detailed therapeutic documentation.

  • Must obtain CPR and First Aid certification within first three (3) months of employment.

  • If transporting people in services must present current MVR before hiring.  After hiring, must present current driver's license and proof of auto insurance.
