Talbert House Therapist / Social Worker (LSW / LPC) - Healthcare for Homeless Veterans Program (HCHV) in Cincinnati, Ohio

Participate in interdisciplinary treatment | | planning for all applicable clients.  Develop comprehensive | | diagnostic assessment within the scope of license. | +-----------------------------------------------------------------------+ | Position Description: | | | | - Develop and/or implement individualized client service plans, | | monitor progress utilizing supportive methods, and determine | | updates needed in collaboration with the client and team during | | weekly case conferencing. | | - Work with offsite support, coordinate and provide care that is | | safe, timely, effective, efficient, equitable, and | | client-centered according to plans developed in conjunction with | | our partners at the VA. | | - Deliver on-going services to clients which may include: crisis | | intervention, advocacy, transportation assistance, budgeting, | | housing assistance, and referral and linkage to community | | resources as needed. | | - Assist in helping clients achieve wellness and autonomy through | | supportive counseling and facilitating multiple care aspects | | (care coordination, information sharing, etc.). | | - Maintain caseload and productivity by maintaining regular contact | | with clients to monitor and assess progress. | | - Document all clinical activities to meet specific program, funder | | and credentialing entity criteria timely and with accuracy. | | - Ensure compliance with applicable standards, agency policies, and | | contractual requirements. | | - Adhere to professional standards as outlined by protocols, rules | | and regulations. | | - May require transportation of clients in vehicle. | | - Other duties as assigned. | +-----------------------------------------------------------------------+

Qualifications

+-----------------------------------------------------------------------+ | Required Knowledge, Skills and Abilities: | | | | - Ability to interact with others in an ethical, culturally | | sensitive, and respectful manner. | | - Be adaptive, flexible, and responsive to the needs of the client | | and program. | | - Demonstrate professional behavior and strong clinical boundaries. | | - Ability to teach clinical therapy principles. | | - Excellent organizational and time management skills. | | - Effective verbal and written communication skills. | | - Advanced problem-solving skills and ability to multi-task while | | prioritizing tasks. | | - Remain current in general knowledge of psychotropic medications, | | side effects, and DSM/IC diagnoses. | | - Ability to use electronic service documentation systems | | effectively. | | | | Job Requirements: | | | | - Bachelor's degree or higher in Social Work or a related field and | | at least one of the following licenses: LSW, LPC  | | - 2 -- 5 years clinical counseling experience preferred | | | | Essential Functions/Physical Demands: | | | | - Positional: Driving in accordance with job duties assigned. | | Infrequent standing and walking. Frequent to constant sitting. | | - Gross Mobility: Rare climbing, or crawling. Infrequent balancing, | | stooping, kneeling, or crouching. Frequent to constant reaching | | and handling. | | - Sensory: Rare tasting/smelling. Infrequent use of color vision. | | Occasional use of far visual acuity, depth perception, and field | | of vision. Frequent use of near and midrange visual acuity, and | | visual accommodation.
