Job Overview: The Center Manager for PACE of Cincinnati has leadership responsibilities for the operation of the PACE of Cincinnati PACE Center and critical PACE Interdisciplinary Team (IDT) functions. This includes supervisory responsibility for key IDT staff, management oversight of the PACE Clinic (in collaboration with the Medical Director), management responsibility to ensure the PACE Center is safe, clean, and functional for participants, staff, and visitors, managing all critical functions of the IDT, working with the Intake Department to ensure an efficient enrollment process for new participants, and participating as a member of the PACE leadership team. Job Requirements: Bachelor's degree in a health-related field required; RN, Social Work and/or Master's preferred. Either have one year of experience working with a frail or elderly population or, in the absence of such knowledge, receive appropriate training from the PACE of Cincinnati on working with a frail or elderly population upon hiring. Experience working in Electronic Health Records (EHRs). Before working independently, meet a standardized set of competencies for the Center Manager position established by PACE of Cincinnati and approved by CMS and ODA. Before engaging in participant care, be medically cleared for communicable diseases and have up-to-date immunizations and vaccines. Not been convicted of criminal offenses related to involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act. Have not been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse. Have not been excluded from participating in Medicare and Medicaid programs. BLS certified. Three (3) years of experience in a supervisory capacity in a healthcare setting. Job Responsibilities: Responsible for supervising PACE Center staff as dictated in the PACE of Cincinnati organization chart. Supervisory responsibilities may include, but may not be limited to, nursing staff, physical and occupational therapists, social worker staff, home care coordinators, transportation staff, recreation staff, dietary staff, PACE Center aides, and PACE Center support staff. Responsible for coordinating all IDT activities, including participant care planning, the Service Determination Requests (SDRs) process, the Appeals procedure, the Grievance procedure, daily morning meetings, IDT meetings, and coordinating and leading participant/caregiver conferences. Responsible for timely coordination of all PACE voluntary and involuntary disenrollment activity. Responsible for coordinating 24-hour care delivery to PACE of Cincinnati participants. Responsible for overseeing PACE participant quality of care and safety at the PACE of Cincinnati Center and in the participant's home in collaboration with medical director, clinical providers ( physicians and nurse practitioners). Responsible for ensuring participant and caregiver satisfaction with services provided through PACE of Cincinnati. Responsible for ensuring participant rights and responsibilities are upheld for PACE of Cincinnati participants and caregivers. Be available or engaged with the PACE Participant Advisory Group Responsible for leading and managing all activities related to the day-to-day operation of the PACE Center to ensure a safe, clean, and functional environment for participants, staff, and visitors. Responsible for managing the PACE Center supply chain to ensure each area has the necessary supplies to meet participant and staff needs. Responsible for PACE Center emergency preparedness activity, including ensuring necessary equipment is present and in working order; Responsible for ongoing PACE Center emergency preparedness monitoring per applicable federal and state laws and regulations.
