The Ambulatory Triage Nurse is primarily responsible for receiving and triaging incoming clinical calls for various UC Health Ambulatory departments. The Ambulatory Triage Nurse is responsible for medical triage of calls from patients who are presenting with symptoms using established protocols and providing specific guidance based on these protocols so that the patient is advised of the appropriate level of care. The Ambulatory Triage Nurse plays an integral role in the management of patients? care in the ambulatory setting and is expected to collaborate with UC Health providers and staff members to ensure safe, timely, and effective patient care.Education: Bachelors Degree, Nursing is Required, Masters Degree Preferred License/Certification: Ohio Registered Nurse (RN) license required to be hired Kentucky and Indiana Registered Nurse (RN) license required within 30 days of hire Healthcare Provider CPR Certification Experience: 3 - 5 Years equivalent experience - Minimum Required, 2 Additional years equivalent acute care experience - PreferredPATIENT POPULATION - (CLINICAL ONLY) 5% Engages in population appropriate communication. Has knowledge of growth and development milestones and tasks. Gives clear instructions to patients/family regarding treatment. Involves family/guardian in the assessment, initial treatment and continuing care of the patient. Identifies any physical limitations of the patient and deploys intervention when necessary. Recognizes and responds appropriately to patients/families with behavioral health problems. Interprets population related data and plans care appropriately. Identifies and responds appropriately to different needs resulting from, unique psychological needs or those associated with religious / cultural norms. Performs treatments, administers medication or operates equipment safely. Recognizes and responds to signs/symptoms of abuse or neglect. Inbound Clinical Calls -Handles inbound clinical calls from symptomatic patients. Adheres to established policies and procedures for the nurse triage process. Maintains and expands knowledge of telephonic triage processes and clinical expertise. Provides Telephone Triage - Provides telephone triage using Schmitt-Thompson protocols with in the Epic Nurse Triage software. Aligns the right level of care for the patient within the right timeframe and with the right provider. Adheres to guidance produced by protocols within Epic software. Documentation - Appropriately documents call within patient's chart in Epic. Documents triage questions and resulting disposition. Routes encounter to appropriate pool, provider, or staff member per department protocols. Team Collaboration - Partners and collaborates with patients, family members, physicians, and personnel as an integral part of the patient care team. Provides clear communication and follows established procedures to ensure successful functioning of the interdisciplinary team. Patient Advocate - Acts as a patient advocate by solving problems or communicating patient needs and concerns to appropriate resources. Evaluate patient's needs and address or escalate per departmental protocols.
