Amergis Manager of Complex Care Coordination in Chicago, Illinois

$50 / hour

Position: Manager of Complex Care Coordination

Location: Chicago, IL

Pay Rate (Local): $50/hour

Schedule Options:

  • 8am-4:30pm Monday to Friday

Weekend Requirements: None

Duration: 3 months with extension based on performance

Responsibilities:

  • Maintains full caseload, complete with ongoing individual service plans

  • Assess ongoing changes in behavior, circumstances, or conditions that may affect patient safety

  • Engages /involves client and sponsor in the casework process

  • Prepares, compiles, and submits accurate case files

  • Serves as a liaison with stakeholders (i.e.. legal providers, immigration court)

  • Consults with supervisors to determine decisions as they relate to the safety, well-being and permanency of patient

  • Maintains accurate records, files, forms, statistics and additional relevant information in accord with agency policy, licensing, and/or funding requirement

Minimum Requirements:

  • Bachelor's degree or higher from an accredited college or university

  • Must be license in the state of Illinois as a RN, LCPC, LSW LCSW or LPC

  • 3 years experience of Care Coordination work experience in health services or social services

  • 2 years of experience working for a culturally diverse population

  • 1 year of supervisory work experience with a multi- disciplinary team

  • Prior case management software work experience

  • Must have a valid driver's license and access to an insured vehicle

  • Covid-19 Vaccine required
