Work Schedule: 8 Hr (8:30:00 AM - 5:00:00 PM)

Position Highlights:

  • This is a newly created role that will allow for integration between an outpatient addiction medicine clinic, the inpatient substance use intervention team and the inpatient consult service.

  • Develop a system for continuity of care for inpatients and outpatients with co-occurring substance use disorders

  • Act as a clinical liaison for patients with co-occurring substance use disorders across multiple medical services, clinical departments and facilities.

  • Serve as a call center contact and escalation point for intake coordinators

  • Provide injections in an ambulatory clinic.

The Clinical Nurse Navigator will provide care coordination, follow up, and support to patients in an effort to improve their health outcomes. Exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

Other information:

• Current State of Illinois Registered Nurse licensure required.

• Bachelor’s degree in Nursing or Master’s degree in Nursing required.

• At least three years of registered nurse experience required.

• BLS certification required.

• Excellent written and verbal communication skills.

• A high level of interpersonal skills is required.

• Proficiency with the Microsoft Office Suite.

• Data entry skills with attention to details.

• High degree of accountability and reliability.

• Ability to adaptable to change and interact with a variety of providers with different protocols and care processes.

• Ability to work independently as well as to integrate as part of a team and part of clinic flow.

• Cultural competency expressed via experience and interest in working with (culturally, economically and educationally) diverse populations, along with ability to work in low resource environments.

• Must be a resourceful and resilient, innovative problem solver and motivated learner. Friendly, cooperative, passionate and compassionate.

Responsibilities:

• Maintains and ensures compliance of all policies and procedures that guide and support the assigned services. These include, but are not limited to Joint Commission requirements, federal and state regulations, regulatory standards, etc.

• Provides consultation in specialty area to all levels of health care providers throughout the organization, affiliated schools of nursing, and the community.

• Offers guidance and emotional support to patients regarding decisions on follow-up and treatment plans.

• Arranges appointments with patients, reviews test preparation procedures, and assesses potential patient barriers to ensure the patient completes testing process.

• Contacts patients via phone calls, emails and letters to remind them about appointments, assists in scheduling/rescheduling appointments and answers questions about testing and treatment.

• Performs follow-up phone calls to patients after appointments as indicated.

• Works with participating sites to develop mechanisms for the sites to share the patient data (for those requiring follow-up) in a manner that utilizes their existing data and optimizes their time and effort.

• Completes all necessary electronic records and paperwork regarding patient interactions and appointments.

• Develops and maintains relationships with community leaders, community-based organizations, faith-based organizations, health institutions, schools, daycares, etc.

• Attends community-based events, including health fairs, support group meetings etc.

• Assists with the planning and execution of forums, special events and presentations to reach and educate the community.

• Supports reporting and paperwork to support funding and grant requirements, including tracking patient metrics such as appointment no-shows and lost to follow up ratios.

• Tracks all outreach and navigation activities and provides monthly reports.

• Demonstrates an awareness of self-responsibility and accountability for own professional practice and development.

• Administers nursing care to patients if applicable including assessment, planning, implementing, and evaluating the patient's care plan in coordination with the physician.

• Provides orientation/education for center and hospital staff, students, and other hospitals and community organizations, as requested.

• Performs related duties as required.
