Job Summary
The Licensed Practical Nurse assists in collecting patient information, planning, and implementing patient care under the direction of a Registered Nurse. The position is accountable for all delegated patient care based on established standards of nursing practice and hospital policies and procedures. The LPN only performs those procedures and processes that are within the scope of practice for an LPN.
Essential Duties and Responsibilities
These duties and responsibilities described below represent the general tasks performed on a daily basis; other tasks may be assigned.
Patient Care Management:
Collect/gather available patient data; review collected data with supervising RN
Routinely utilize the standard of care as the basis for nursing care and documentation
Document individual patient/family teaching
Implement plan for discharge upon admission and throughout the hospitalization
Consult with and keep Clinical Coordinator and Charge RN informed of unit activities, needs, and problems
Organize and prioritize work based on staff needs, patients' needs, available resources, and unit requirements
Actions demonstrate sound judgment and decision making skills in all situations
Demonstrate efficiency in prioritizing patient needs
Safely administer medication in keeping with LPN scope and organizational and unit policy and procedure.
Demonstrate consistent ability/competence in patient care areas
Actively solicit feedback from patients and visitors
Recognize limitations and seek the appropriate assistance when needed; function within the scope and practice of an LPN
Demonstrate ability to respond to emergency situations
Accurately document patient care per hospital policy
Provide a safe environment for patients and visitors
Contribute to reduction of infections and adhere to infection control guidelines
Foster spirit of teamwork within assigned area
Participate in quality improvement activities within the unit

Supervisory Responsibilities
Reports to: Nurse Manager
Supervises: Unlicensed Assistive Personnel (UAP)

Education and Experience
Minimum Required Education: Successful completion of an accredited Licensed Practical Nursing program
Preferred Education: Not applicable
Experience: One (1) or more years’ previous acute care experience
Certificates, Licenses, Registrations
Applicant must be licensed to work as a Licensed Practical Nurse (LPN) in the Commonwealth of Virginia. BLS Certification within 90 days of hire.
