Job Description:

Reasonable for support functions for the care management team. Assists in the coordination of care and management of caseloads including, conducting patient home visits to evaluate plan of care and ongoing community service needs. Serves as a resource to staff and others, provides a wide range of supportive office functions for the office-based team.

Minimum or Preferred Qualifications:

AAS - Associate of Applied Science required. BS - Bachelor of Science preferred. 2 years of experience in human services required. 2 years of experience in a homecare setting preferred. NYS Drivers License upon hire required.

Salary Range:

$35,548.50-$48,886.50 annually Wage will be determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement.
