University of Virginia Assistant Nurse Manager Case Management in Charlottesville, Virginia

A transformational leader responsible for supporting the nurse manager in administrative duties of the department. The Assistant Nurse Manager will provide leadership by collaborating with members of the health care team to maintain standards of care around the discharge process for the populations served by the organization. Participates in organizational and department quality improvement activities, and provides input on standards of care. Must provide discharge planning on an as needed basis and maintain competencies with the potential for weekly staffing coverage.

  • Provides first line supervision of RN Case Managers and support staff within the inpatient setting.

  • Performs case manager duties in accordance with established policies, procedures and standards of practice.

  • Performs initial assessment on all patients. Begins discharge process from day one of patient’s stay.

  • Communicates with payers to facilitate the patient’s effective and efficient movement through the hospital system.

  • Complete level of care assessment to ensure patients are receiving the appropriate level of care and to ensure that the patient transitions through their stay without barriers or delays in care delivery.

  • Work closely with the Admitting Physicians to ensure appropriate documentation of clinical information necessary to support or determine alternative level of care.

  • Coordinates discharges home and facilitate transfers to external placement facilities.

  • Maintains expert knowledge of Interqual Level of Care criteria.

  • Oversees care provided by maintaining high visibility to patients/families and staff in the clinical area(s).

  • Share accountability with manager for department/organizational goals achievement including patient experience, team member engagement and continuous process improvement.

  • Staff management to include recruitment, retention, coaching, training, evaluation and development, as well as preparation and delegation of work schedules and workflow

  • Under supervision from manager, participates in basic human resource functions.

  • Maintains a positive and professional work environment.

  • Is accountable for outcomes.

  • Identify patient’s readiness to discharge based on outcomes.

  • Review all regulatory requirements to ensure high quality safe and cost-effective care.

  • Identify patients with high-risk concerns for presentation at Clinical High-Risk meeting based on experience high risk report or denials.

  • Evaluate and measure outcomes in order to continuously improve practice and to enhance outcomes.

  • Maintains all records pertaining to the staff competency and quality audits and other related records.

  • Serves as a professional role model, coach, and resource for employees. Participates actively in Nursing and/or hospital committees and work groups and assists in the development and implementation of the units shared governance model.

  • Assumes responsibility for own professional growth.

  • In addition to the above job responsibilities, other duties may be assigned.

Position Compensation Range: $104,062.40 - $138,819.20 Annual

MINIMUM REQUIREMENTS

Education: Bachelor’s of Science in Nursing required.

Experience: 5 years clinical nursing experience including 3 years case management experience required.

Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Case Management Association or Commission for Case Manager certification required within 1 year of hire.

PHYSICAL DEMANDS

Job requires standing for prolonged periods, frequently walking, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
