Description:

The Community Hospice has a full time opportunity for a social worker to join our hospice care team caring for patients in the Columbia and Greene County area as part of our Interdisciplinary Team caring for patients and families receiving hospice care.

As a member of the hospice interdisciplinary team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff. Position is Part Time 8a-4p.

Responsibilities:

  • Diagnose and treat psychological, behavioral and emotional disorders as they relate to end of life care

  • Provide specialized skills and mental health therapy for clients

  • Identify barriers which impact optimal patient functioning

  • Ensure patients receive appropriate resources in most appropriate setting

What you will need:

  • NYS SW Licensure

  • Valid NYS Drivers license

  • Minimum 2-3 years experience ideally caring for seniors or those with advance illness

Pay Range: $29.00 - $41.40

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
