Dignity Health Mercy San Juan Medical Center is a 384-bed not-for-profit Level 2 Trauma Center located in Carmichael California. We have served north Sacramento County as well as south Placer County for over 50 years. Our facility is one of the area's largest medical centers and also one of the most comprehensive. Responsibilities
The incumbent works in collaboration with the Medical Director, patient physicians, and team members, in an active practice to deliver and support appropriate clinical care under the Hospice/Palliative Care Benefit according to regulatory, quality, and accreditation standards. Participates in the assessment, planning, implementation and evaluation of palliative and primary care interventions. Practices according to organizational policies and procedures to ensure excellence involving all aspects of the patient's clinical care and outcomes.
Mercy Hospice is a comprehensive program offered by Dignity Health for individuals and families facing serious and life-threatening illness with a prognosis of six months or less. Mercy Hospice professionals and volunteers offer physical, emotional and spiritual comfort to patients and families. The primary focus of Mercy Hospice is to enable families to care for their loved ones at home. Specialized support and care are carried out under the direction of a physician and include:
Regular nursing visits to assist in pain and symptom management
Emotional support education and community resources provided by a social worker to assist the patient and family
Spiritual support provided by a hospice chaplain
Personal services provided by certified home health/hospice aides
Volunteer services for companionship respite and caregiver support
Continuing support to families for a period of 13 months through our bereavement (grief) program
The Mercy Hospice team consists of physicians, nurses, social workers, counselors (including dietary) chaplains, home health aides and volunteers. The hospice team meets regularly to discuss current cases and coordinate care to ensure quality and continuity for the patient. The medical director meets with the team to review care plans giving direction and advice as necessary. Additional services are provided such as pharmacy physical therapy and occupational therapy when needed.
Hospice provides 24 hour-a-day support for patients and their families.
Mercy Hospice is a Medicare-certified Accreditation Commission for Health Care accredited programs. We are also a member of the National Hospice and Palliative Care Organization (NHPCO) California Hospice and Palliative Care Association (CHAPCA) California Association for Health Services at Home (CAHSAH) and the National Association for Home Care & Hospice (NAHC). We are a partner in the We Honor Veterans (WHV) program. Mercy Hospice also contracts with many major private insurance companies that provide a hospice benefit. Regardless of ability to pay Mercy Hospice is available to patients and families who are facing life-limiting illness.

Provide competent compassionate care when it matters most. The incumbent provides care treatment and service to Hospice patients and their families in their place of residence. Functions as a member of the Interdisciplinary Team.
Qualifications
Minimum:
2 years experience as an RN in Home Health, Hospice or Palliative Care setting
Nurse Practitioner experience in Hospice preferred
Current CA NP license, NP Furnishing Number and DEA Number
Current AHA BLS
Current CA Driver’s license and car insurance
AHA ACLS required for credentialing
Preferred:
- Certified Hospice and Palliative Care Nurse (CHPN)
Pay Range
$64.51 - $90.38/hour
