Job Brief

The QIDP coordinates all aspects of care for residents in order to ensure active treatment is provided on an on going basis in the least restrictive environment, in accordance with the OPWDD.

25.64 - 28.21

The QIDP coordinates all aspects of care for people we serve to ensure active treatment is provided on an ongoing basis in the least restrictive environment, as per the OPWDD.

The QIDP provides oversight, coordination, supervision, and evaluation of operations of assigned programs/sites, including site supervision for therapeutic or counseling professionals that provide services.

Physical Expectations

This position requires that employees are able to stand and or sit for prolonged periods of time. The employees must also be able to lift and transport consumers and perform all physical aspects of the job.

Essential Duties and Responsibilities include but are not limited to the following:

· Ensure all individuals’ services are coordinated, integrated, and implemented properly between CPU programs (Day Hab/Residential/Com Hab, etc.).

· Coordinate individuals’ services with Care Manager and other outside service providers.

· For ICFs: Prepare annual CFA’s and summaries, Coordinate/conduct all IDT meetings – ensure completion and receipt of necessary reports 2 weeks in advance

· For IRAs: Attend Life Plan meetings, Write/certify monthly notes, Staff Action Plans, etc.

· Ensure the follow-up of recommendations stemming from reviews/special meetings.

· Ensure least restrictive interventions or practices are utilized and Behavior Support Plans that contain restrictions/limitations are approved by the HRC

· Develop and implement goal programming and activity schedules.

· Monitor data collection on an ongoing basis.

· Ensure individuals’ personal environment is clean, safe, and well-maintained.

· Ensure house systems support quality care and treatment

· Ensure individuals’ finances and personal property are appropriately managed and protected.

· Conduct observations to ensure quality services – provide hands-on training as needed

· Ensure incidents and occurrences are reported and documented as needed.

· Participate in ICF, DQI, and OFPC audits.

· Ensure staff schedule supports individuals’ needs, programming and levels of supervision

· Ensure staff are appropriately trained and supervised – conduct trainings as needed/assigned

· Counsel/discipline/supervise/evaluate staff as needed/assigned

· Assist in investigations as requested/assigned

· Responsibly report any violation of agency policy/procedure, and/or Federal or State Laws.

· Complete QI reports as needed and ensure resolution of outstanding issues.

· Obtain/maintain agency van certification and transport as needed

· Obtain/maintain AMAP certification and administer medications as needed.

· Maintain communication with family/correspondent

· Model and ensure staff interactions support the core values and mission of the Agency.

· Other duties as needed or assigned by supervisor or authorized administrator.

JOB QUALIFICATIONS

Education: Bachelor’s Degree in related field of study required.

Required Experience: Supervisory experience preferred

At least one year’s experience in the field.

Well-organized, experience writing formal goals. AMAP certified (or ability to obtain certification w/in 3 months of hire) Agency driver authorized (or ability to obtain authorization w/in 3 months of hire)
