Hildebrand Self-Help Center Bilingual Stabilization Case Manager (Haitian Creole) in Cambridge, Massachusetts

We are looking for people to partner with us to achieve our goal of disrupting the cycle of homelessness for greater Boston families and are seeking a Stabilization Case Manager to join our team.

What You Will Do:

  • Support families exiting Hildebrand Emergency Shelter for 24 months.

  • Enter a family’s initial stabilization plan and progress into ETO/ASIST and update every 30 days until the stabilization services have ended.

  • Have at least a monthly communication with the landlord to confirm that the family is paying rent and to identify any issues that have arisen.

  • Document stabilization efforts and meetings utilizing the EA Case Notes in ETO/ASIST.

  • Track and record client’s demographics, income, immigration status, Hildebrand tiering system and other key measurable data points as requested on a weekly and monthly basis.

  • Cultivate successful landlord-tenant relationships, including monthly communication, and provide mediation if necessary.

  • Advocate for each family in any housing court appearances.

  • Intervene if the tenancy is endangered, for any reason.

  • Assist families in securing appropriate and adequate medical services and benefits with a primary health care provider.

  • Assist in the development and facilitation of groups, classes, and skills training to help clients in Emergency Shelter and Stabilization achieve goals and specifics within the action plan set.

  • Measure and review case progress and facilitate case consultation meetings.

  • Work as part of a team and coordinate with Family Shelter Case Managers, Housing Specialists, System Navigators, supervisors and other program staff.

  • Attend weekly supervision, weekly staff meetings, in-service training, and team meetings.

  • Participate in on-call duties.

    What You Will Need to Bring:

  • Education – Associate Degree, Bachelors preferred. A combination of education and experience will be considered.

  • Experience – 3 to 4 years of experience in case management, housing search or related work with homeless or low-income families. Detailed understanding of Massachusetts benefit programs, state and federally funded housing programs, private housing developments and the social service network. Residential program experience is a plus.

  • Skills – Ability to work effectively with individuals from a wide range of ethnic and social backgrounds is critical. Must be computer literate, familiar with MS Word and competent to utilize a variety of software programs necessary for reporting requirements. Must be able to communicate effectively verbally and in writing. Ability to ambulate stairs unassisted to conduct home visits.

  • License/Certification – Must be CPR/SFA Certified within 30 days of hire

  • Language – Bilingual in Haitian Creole required

  • Additional Information - Use of a personal vehicle or ability to efficiently utilize public transportation for home visiting is required.
