Summary:

The Clinical Transformation Project Coordinator reports to the Manager of Clinical Transformation and Physician Relations. This position is accountable for the daily management and coordination of a variety of different initiatives/programs within the Catholic Medical Partners, IPA. This position works collaboratively with different departments within CMP and Catholic Health (CH) in developing, administering and integrating project work. This position will develop and drive completion of project plans that outline specific requirements, milestones, goals, timeline and reporting lines.This position has the ability to take on new initiatives as needed via CMS, NYS or through new grant work. Current expected responsibilities include but not limited to:

  • Managing rollout and training program for CMP Population Health Tools.

  • Providing leadership and acting as key liaison for CMP practices participating in government programs (e.g. ACO REACH, PCF, etc) to support the achievement of performance goals including developing training materials, and coordinating group, web-based, and in practice training as needed/requested.

  • Lead health plan payer projects and serve as the subject matter expert in these areas.

    This role will support the CMP leadership team by researching and staying on top of new CMS, NYS or other grant opportunities that pertain to Catholic Medical Partners, IPA and other key stakeholders. The position works closely with the Catholic Medical Partners IPA leadership to ensure organizational awareness and compliance with all aspects of project requirements.

Responsibilities:

EDUCATION

  • Bachelor's degree required; preferably in a healthcare related field

  • Project Management Professional (PMP) Certification preferred

EXPERIENCE

  • Minimum of six (6) - ten (10) years of relevant work experience in a healthcare related background

  • Previous experience working with CMS or other governmental agencies

  • Previous EMR experience with Medent or eClinicalWorks required

  • Previous experience Quality and National Recognition Programs (i.e. PCMH, PQRS, NQF) required

  • Knowledge of the accountable care model

  • Previous project management experience

KNOWLEDGE, SKILL AND ABILITY

  • Previous Project Management experience strongly required

  • Excellent verbal and written interpersonal communication skills required

  • Knowledge of MACRA, MIPS, ACO REACH

  • Excellent Microsoft Word, Excel and PowerPoint software application skills required

  • Demonstrate ability to quickly learn, apply and teach new requirements, programs to various stakeholder groups

WORKING CONDITIONS:

ENVIRONMENT

  • Associate must have reliable transportation and be willing to travel on a regular basis up to two (2) hours from CMP office location to meet with CMP members Normal heat, light space, and safe working environment; typical of most office jobs
