Family Help Center is seeking a full-time High Fidelity Wrap Care Coordinator for an open position in the wraparound preventive service program targeting families in the Erie County Children's System of Care. Care Coordinators provide home-based Services to assist youth and families with strength-based service planning, providing linkages, coordination, and monitoring of services to improve family functioning, ensure safety of children, and reduce the risk of out of home placement of children.
Minimum Requirements: Master's degree in social work (or other-related field- Human Services, Sociology, Psychology, Education, Criminology) or a Bachelor's Degree in Social Work or related field is required with a minimum of two years direct service experience working with high-risk, complex needs. Preferred knowledge of trauma informed care and impacts of trauma on children and families.
Care Coordinators are responsible for convening child and family teams from which a plan of care is developed for each family. Care Coordinators are responsible for conducting regular in-home visits, coordinating, monitoring, and maintaining in-home vendor services, required documentation, collateral contacts, and timely progress notes.
Candidates should be organized, able to multi-task, be dependable, and compassionate. The ability to meet deadlines and work effectively with families, staff, and other agencies from a variety of cultural and diverse backgrounds is needed.
Education: Bachelor's degree (required) with 2 years relevant experience; Master's degree preferred.
