Your primary role will be to assist prospective patients with enrolling in and beginning to receive home care services. This includes introducing and explaining home care services and eligibility to prospective patients, as well as educating and guiding them through the enrollment/onboarding process. You must understand the importance of completing tasks on-time and staying organized while simultaneously providing valuable assistance and customer support to our future consumers. A friendly attitude and a willingness to help those in need are essential.Responsibilities:Staff caregivers to assigned patients using our software-based systemDocument all notes and tasks in the system accurately and timelyVerify attendance with both patient and caregiverReport and document any incidents / concerns to ManagementParticipate in On-Call Hours as scheduled (Bonus Reimbursement available!)Maintain accurate schedule data in the software-based system and adjust schedule as neededFollow guidelines and compliance procedures implemented by Ideal Home HealthProactively resolve, manage, communicate, and address matters related to patients and/or caregiversQualifications:Strong oral and written communication skillsAbility to read, interpret, and communicate basic math and dataFluency in Spanish and EnglishProven ability to multitask, prioritize, and meet deadlinesBasic knowledge of using a computerBenefits:Employer funded HealthcareCommuter Benefits ProgramJob Type: Full-timeSchedule: 9-5 M-FSalary Range: $17-$20/HRAdditional Information:NOTE: This job description is intended to describe the general requirements for the position.
