Purpose of this Position
The Single Stop Case Manager (SSCM) is responsible for pre-screening Win clients in order to ensure residence eligibility for legal, financial and/or intensive case management services in the Single Stop Program. The SSCM will work collaboratively with transitional housing case management staff to schedule sessions with clients who need intensive Single Stop case management services.
Responsibilities:
Determine client eligibility for services from the Single Stop Program.
Assess the service needs of clients and refer them to the appropriate service providers.
Counsel and assist clients in obtaining entitlements services.
Escort Win families to court proceedings and appointments.
Collaborate with the shelter staff to ensure that services are effectively coordinated.
Coordinate legal and financial service providers to meet with clients.
Prepare and maintain accurate and complete reports as required by the Robin Hood Foundation and Win.
Commit to using evidence based practices (EBP); strength-based case management, motivational interviewing, trauma-informed care, critical time intervention, family-centered case management, boundary setting, and cultural-reflexivity to help clients achieve their goals.
Provide continuous feedback to the Program Director to ensure the Single Stop Program meets the needs of the clients.
Attend and participate in on/off site meetings, trainings, and case conferences as directed by the Program Director.
Act as a member of the transitional housing multi-disciplinary team.
Coordinate and facilitate client workshops.
Perform other related duties as assigned.
Essential Functions:
Must be able to ascend/descend stairs in an eight (8) story building.
Must be able to escort clients to appointments using public transportation.
Qualifications:
Commitment to Win’s mission, vision, and values.
BA degree with two (2) years of case management experience.
Knowledge of homelessness issues and at-risk populations.
Proficient in Microsoft Office Suite.
Excellent communication and writing skills.
Bilingual - English/Spanish a plus.
