The Mishkon Division provides supports and services in a healthy and safe environment for individuals with intellectual/ developmental disabilities living in residential programs and in community settings. Our core values are to provide a healthy and safe environment that promotes person centered planning, community integration, and close collaboration with families and the community. This is all accomplished through an Orthodox Jewish lens, and with an understanding that staff, at all levels of the division, is the determining factor in achieving positive outcomes for our clients.

POSITION OVERVIEW:

This position is responsible for the development, implementation, and facilitation of a Person-Centered-Plan process for individuals served. This includes monitoring active treatment in accordance OPWDD and CMS regulatory standards and the facility's policies and procedures.

KEY ESSENTIAL FUNCTIONS:

  • The QIDP is responsible for the development and implementation of individual programming for the individuals served together with the other members of the Interdisciplinary team.
  • The schedule is expected to be varied to meet the needs of the individuals and residences.
  • Assess, create and track valued outcomes/goals.
  • Participate as an active member of the interdisciplinary team process.
  • Coordinate and participate in annual Human Rights and Informed Consent Committee.
  • Interact with the residential individuals on a daily basis.
  • Record each individual's progress via monthly notes, 30 Day, Semi-Annual and Annual Case Conference Reviews.
  • Attend Psychiatric appointments and day program meetings for the individuals on their caseload.
  • Initiating periodic reviews as necessary.
  • Train staff in in the areas of social issues along with instruction goals/valued outcomes, and various clinically required trainings as per state regulations.
  • Coordinating services and the delivery of each resident's individual plan of care.
  • Integrating the various aspects of the facility's program and clinical services.
  • Keeping active communication with families and outside providers as it pertains to the individual's level of care.
  • Be a strong advocate for the individuals and work in collaboration with various other disciplines to ensure optimal care.
  • Complete observations and monthly documentation as required.

CORE COMPETENCIES:

  • Understanding of OPWDD and DOH regulations and standards.
  • Must be an effective writer and speaker and able to coordinate services.
  • Must be able to maintain proper records and documentation.
  • Must have an understanding of Goal assessments and implementation.

EDUCATIONAL / TRAINING REQUIRED:

  • BA/BS in a Human Services related field. Two years' experience in working with individuals with developmental disabilities or related field. Candidate needs to have effective communication skills in both written and oral presentation. They must have the ability to manage multiple priorities.

COMPUTER SKILLS REQUIRED:

  • Must be organized with proficient use in word and excel.
  • Must be skilled in areas of documentation and report writing.

VISUAL AND MANUAL DEXIERITY:

  • Must be able to review IPP's and all treatment plans and document manually or via computer daily logs.
