Our client, A Leading Health Care Agency, is seeking a highly motivated and experienced Intake LPN to join their team. The Intake LPN will be responsible for supervising, instructing, and guiding intake coordinators and auxiliary personnel in the referral intake process. This role involves designing and implementing efficient work processes to ensure seamless communication between departments, referral sources, and physicians. Play a key role in ensuring efficient referral processes that enhance patient care and satisfaction.
Key Responsibilities:
Supervision & Leadership: Supervise, instruct, and guide intake coordinators and auxiliary personnel to ensure high-quality referral intake and follow-through.
Process Design & Implementation: Develop and implement efficient work processes to facilitate sound communication between departments, referral sources, and physicians.
Team Management: Provide ongoing support and guidance to the intake team, fostering a collaborative and efficient work environment.
Performance Monitoring: Monitor and evaluate the performance of intake staff, ensuring compliance with organizational standards and implementing improvements as needed.
Communication Facilitation: Ensure effective and timely communication between all stakeholders involved in the referral process.
Qualifications:
- Experience:
Minimum of 2 years of experience in a Certified Home Health Agency (CHHA) intake department.
At least 1 year of supervisory experience in a related field.
- Technical Skills:
Proficiency with Allscripts or similar electronic health record (EHR) systems.
- Additional Skills:
Strong leadership and communication skills.
Ability to design and implement efficient work processes.
Excellent organizational and multitasking abilities.

The salary range for this position is $38-$39.2/hour
