Rising Ground PRS Level 1 - Case Worker in Bronx, New York

Overview

The Passage of Hope (POH) HS/PRS Program provides case management and other supportive services to Unaccompanied Children (UC) who are currently under legal custody of the Federal Government or who have completed the reunification process and are now residing in the community with their sponsors. Working in collaboration with the federal Office of Refugee Resettlement, our HS/PRS program provides support and assistance to families in need of Home Study Services, and families in need of Post Release Services (PRS). For PRS Services we link children and their families with an array of community services that will help them achieve success and permanency in the United States so they are able to move forward with their lives and thrive.

Responsibilities

PRS Job Duties (Levels 1 -3):1. Conduct thorough assessments to ensure the safety and well-being of the child.

  1. Ensure that sponsors and UC participate in all services outlined in the HS recommendation, and any other as needed.

  2. Connect sponsor and child to community resources and coordinate services in the areas of placement and stability; immigration proceedings; guardianship; legal services; education; medical services; individual mental health services; family stabilization and counseling; substance abuse; and gang prevention, as needed and appropriate for the specific child and/or sponsor.

  3. Connect with stakeholders involved in the child’s case including school, legal service provider and extracurricular activities to ensure services are individualized and target the child needs.

  4. Conduct ongoing needs assessment of the child, provide comprehensive case management, and access to therapeutic supports.

  5. PRS-only workers will carry a minimum caseload of 15, if regionally based, or 8, if cases require travel outside the area. Intensive case managers will carry a minimum caseload of 3 cases per worker.

  6. Complete all the assessments and home visits as mandated and as needed:

    a. PRS Level 1- three virtual visits at day 7, 14, and 30 of child’s release

    b. PRS Level 2- Initial Home Assessment within 14 days of release and monthly in-person

    visits

    c. PRS Level 3- Initial Home Assessment within 7 days of release and weekly in-person visits for the first 45 to 60 days, depending on the needs of the family. Following this, weekly contact will be replaced by monthly in-person or virtual contacts for 6 months following referral acceptance.

  7. Complete Notice of Concern (NOC) form for any concerns about the child safety and wellbeing , make notifications to appropriate investigate agencies and notify ORR within the required timeframes.

    1. Document all engagements and visits via the PRS report form and submit monthly reports.
  8. Submit PRS case closure reports as indicated.

  9. Participates and attends all required program related meetings and conferences.

  10. Works effectively with other professionals in the field.

  11. Maintains knowledge of all local and regional services that can be provided to UC and their families

  12. Supports and participates in the group interaction process.

  13. Some evening and weekend hours coverage

  14. Performs other duties as assigned.

Qualifications

PRS Caseworkers (Level 1 PRS) QUALIFICATIONS:

•Bachelor's Degree in a Human Services Field or related field•One year’s experience working with youth in the social service field•Knowledge of child welfare and immigration related legislation•Excellent verbal, written communication and organizational skills are essential•Proficient in Microsoft Office suite, knowledge of Connections•Bilingual (English/Spanish) a must
