Description We are in search of a Patient Care Coordinator to become an integral part of our team in Bronx, New York. The primary role of this position is to interact with patients on a one-on-one basis within the Emergency Department, screen patients with a provided tool, and refer positive screens to a social work consult. This job offers a short term contract employment opportunity.

Responsibilities:

• Engage in one-on-one interactions with patients within the Emergency Department

• Utilize provided tool to conduct patient screenings

• Identify positive screens and direct them to social work consults

• Document Social Determinants of Health Screening Tool within the Electronic Medical Record

• Provide patients with information on available supportive services as determined by the Emergency Department

• Ensure access to Electronic Medical Record/Altera

• Use software like Allscripts, Banner Ads, Cerner Technologies, CRM, Dentrix Dental Software for various tasks

• Handle duties such as answering inbound calls, authorizations, benefit functions, billing functions, charts - graphs. Requirements • Proficiency in using Allscripts software

• Experience in creating and managing Banner Ads

• Familiarity with Cerner Technologies

• Proficient in Customer Relationship Management (CRM) software

• Demonstrated ability to use Dentrix Dental Software

• Experience in answering inbound calls efficiently and professionally

• Proven track record in managing authorizations

• Understanding of benefit functions within a healthcare setting

• Familiarity with billing functions related to patient care

• Ability to create, read, and interpret charts and graphs efficiently
