Brockton Hospital Certified Diabetes Care and Education Specialist (CDCES) in Brockton, Massachusetts

Position Summary:

The Certified Diabetes Care and Education Specialist (CDCES) is responsible for providing diabetes education for patients with Type 1 and Type 2 Diabetes Mellitus, Pre-Diabetes, and Gestational Diabetes. The CDCES will provide diabetes self-management training and support and teach the diabetes 7 self-care behaviors, adhering to the ADA Standards of care. They will collaborate with endocrinology providers and function as a member of the care team. This integral role contributes to the vision of the endocrinology department: to prevent or reduce acute and chronic complications of diabetes, to reduce diabetes-related hospital admissions, and to improve quality of life for those living with diabetes.

This individual is also responsible for maintaining Diabetes Self-Management Education and Support (DSMES) Program Accreditation with the ADA, for overseeing and implementing collaborative DSMES services, including evidence-based service design, delivery, evaluation, and continuous quality improvement, according to the National Standards for Diabetes Self-Management Education. Certification by the National Certification Board for Diabetes Care and Education is required. The Program Coordinator will have a background in diabetes education and program management that may include academic preparation for diabetes care and education, or experience in the direct care of people with a chronic illness.

This is a full time 40 hour/week position

Responsibilities:

Administrative Responsibilities:

  • Oversees the planning, implementation and evaluation of DSMES services.

  • Responsible for maintaining ADA recognition and participating in the evaluation of the program’s effectiveness through collection of program outcome measurements and CQI.

  • Develop diabetes class curriculum and teaching materials.

  • Administers diabetes education classes reviews program content annually.

  • Serves as a liaison between staff involved with the DSMES program, other Signature Healthcare departments and administration.

    Clinical Responsibilities:

  • Management of patient insulin pump therapy and insulin titration.

  • Continuous glucose monitor administration, interpretation and download, and teaching with patients.

  • Thorough understanding of diabetes-related oral medications, insulin management, and incretin mimetics and their administration.

  • Provides assessment, teaching, and follow-up instruction to the patients that are enrolled in the program.

  • Provides nutrition education and has excellent understanding of a low-carbohydrate diet

  • Performs other duties as assigned.

    Required Knowledge and Skills:

  • Ability to solve practical problems and deal with a variety of variable in situations where only limited standardization may exist.

  • Must be able to appropriately and professionally communicate with patients, physicians, vendors, outside agencies and other colleagues.

  • Organization skills required to effectively deliver and manage DSMES programming.

  • Must maintain continuing education in the areas of diabetes management, behavioral interventions, teaching and learning skills and counseling skills.

    Education/Experience/Licenses/Technical/Other:

  • Education: Bachelor’s degree required

  • Experience (Type & Length): Clinical experience in diabetes management, program planning and management

  • Certification/Licensure: Certified Diabetes Care and Education Specialist. Registered Dietitian or Registered Nurse licensed in the state of Massachusetts

  • Software/Hardware: Computer skills to include Outlook, Word, Excel. Continuous glucometer and insulin pump software.
