Plan, direct, supervises and monitors the activities of the department to achieve optimum patient care by meeting established goals and objectives. Coordinates these activities with the Medical Staff, Administration, other nursing units, families, ancillary services and community agencies to enhance total patient care. May act in the Administrator capacity in absence of the VP/Administrator of WVUHS Home Care.

MINIMUM QUALIFICATIONS IF HIRED PRIOR TO 06/01/2020: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Current unencumbered licensure with the WV Board of Registered Professional Nurses, or appropriate state board where services will be provided, as a registered professional nurse OR current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC). 2. CPR certification within 30 days of hire date. 3. Valid driver's license. EXPERIENCE: 1. Three (3) years of progressive Registered Nurse experience required; 2 years Hospice experience.

MINIMUM QUALIFICATIONS IF HIRED AFTER TO 06/01/2020: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Current unencumbered licensure with the WV Board of Registered Professional Nurses, or appropriate state board where services will be provided, as a registered professional nurse OR current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC). 2. Must possess valid WV or neighboring state driver's license and must maintain WV or neighboring state minimum auto insurance EXPERIENCE: 1. Five (5) years of progressive Registered Nurse experience required; Four (4) years Hospice experience. 2. Two (2) years of supervisory experience.

PREFERRED QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Bachelor of Science in Nursing from a School of Nursing. EXPERIENCE: 1. Two (2) years of previous experience in a hospital/home care/hospice leadership role (such as charge nurse, nurse manager, or documentable consistent demonstration of strong leadership skills while functioning as a staff nurse).

  1. Identifies and details work systems by interviewing customers and identifying prioritizing and having intimate (flowchart) knowledge of systems, outcomes, and monitors. Systems, outcomes, monitors are detailed. Training manuals up to date and complete. Systems are defined, taught, monitored, audited and deficiencies corrected.
  2. Establishes valued standards (quality and cost) and routinely monitors those standards demonstrating incremental measurable improvement identified systems. Standards are measured. Hospice public information is at or above national standards at all times (hospice compare).
  3. Assures that position specific competency based skill evaluations are met within the unit specific orientation period. Competency base education (CBE) complete after orientation.
  4. Provides individualized education plan (IEP) sessions for those Employees who fall below competency level for their job description. Individualized education plan (IEP) current, used up to date.
  5. Requires attendance at mandatory programs with follow-up and discipline for Employees who don't attend. Attendance report.
