Amergis Physical Therapist - 32 hours per week in Bridgeport, Connecticut

The Physical Therapist is responsible for performing patientevaluations, as well as providing and documenting therapy services inaccordance with the plan of care developed for each individual patient andphysician’s orders when applicable.

Essential Duties and Responsibilities:

  • Performs initial comprehensive assessment and reassessmentsand regularly reevaluates patient for rehabilitation needs, and works incollaboration with the physician and other members of the healthcare team tomodify the plan of care as appropriate

  • Participates in developing the plan of care in collaborationwith the physician and other members of the health care team

  • Assesses and evaluates and/or assists the physician inassessing and evaluating therapeutic, rehabilitative, function status, and homeenvironment (as applicable)

  • Assesses muscle strength, mobility, gait, and Range ofMotion (ROM)

  • On incisions resulting from orthopedic surgery, removesstaples or sutures and manages incision line

  • Provides physical therapy treatment and establishes physicaltherapy plan of care

  • Instructs patient, families, and/or caregivers in the use ofand care of therapeutic appliances

  • Determines priority needs for physical therapy

  • Reports patients’ reaction(s) to treatment or changes incondition to physician and/or designee

  • Initiates physical therapy program and instructs, consults,and/or advises other personnel and/or family/caregiver regarding certain phasesof physical therapy with which they may work with the patient, and explains thegoals of the physical therapy program by participating in case conferences

  • Trains patient in use of prosthetic device(s) and use ofother assistive devices (e.g., walker); ensures equipment used in the course ofpatient treatment is in good working order

  • Uses appropriate equipment to meet patient goals, cleans andmaintains equipment according to manufactures guidelines

  • Prepares and submits clinical and progress summaries basedon the attainment and progress of goals, and responses to treatment or changesin condition

  • Participates in discharge planning

  • Participates in peer consultation process

  • Supervises Physical Therapy Assistants according to thecompany’s policies/procedures and state regulations

  • Provides/submits accurate and timely documentation inaccordance with the plan of care and company policy and procedures

  • Complies with applicable local, state, and federal laws andregulations regarding professional standards and practice standards

  • Performs other duties as assigned/necessary

Minimum Requirements:

  • Current licensure as a Physical Therapist in the state ofpractice

  • Graduate of an accredited school of Physical Therapy

  • One (1) year of prior professional Physical Therapyexperience preferred

  • Complies with all relevant professional standards ofpractice

  • Participation and completion of Amergis' Competency programwhen applicable

  • Current CPR if applicable

  • TB Questionnaire, PPD or chest x-ray if applicable

  • Current Health certificate (per contract or stateregulation)

  • Must meet all federal, state and local requirements

  • Successful completion of new hire training as applicable tojob site

  • Understand patient confidentiality and HIPAArequirements

  • Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required

  • Computer proficiency required

  • Must be at least 18 years of age
