Summary:
The Transfer Center care manager will provide leadership in the coordination of patient-centered care across the continuum, and develop a safe transfer plan through collaboration with the multidisciplinary healthcare team. Facilitates episode of care to maximize appropriate level of care, LOS, patient experience, level of care and reimbursement for all patients.
Responsibilities:
Transfer Center Care Manager will be a point of contact for facilities/providers requesting access to services or patient transfer into Northern Light Health.
He/she will obtain patient information critical to determining appropriate, medical necessity and level of care. They will coordinate communication between the referring and receiving facilities.
They will prioritize calls to assure timely access to services within Northern Light Health.
Coordinate orthopedic repatriation patients (including arranging transport back to sending facility).
Evaluate patients for HLOC need.
Arrange procedure transfers for all NL facilities (TEE, endoscopy, surgical procedures, etc.).
Arrange expedited specialist outpatient appointments on stable patients to avoid transfer for consultation.
Triage phone calls from PCP's to ED to determine if there is another appropriate option to get care
Develop working relationships with CM across Northern Light Health and broker transfer back to community hospital during acute phase of hospitalization (if their need from the tertiary center has been met).
Other Information:
Associates degree required; BSN preferred.
Valid State of Maine Nursing License required.
Minimum 3 years of clinical RN acute care experience preferred. Preferred ED or ICU experience.
Preferred care management experience.
