Position Summary:
The Care Coordinator Lead provides oversight to a team of Care Coordinators and maintains responsibility for a reduced panel of members. The Care Coordinator Lead will lead and assist the Care Coordinators with all daily tasks, escalation of member issues as appropriate, and performance management activities. Additionally, the Care Coordinator Lead is expected to be a subject matter expert (SME) for Care Coordinator tasks, and therefore may be asked by leadership to collaborate on workflow development and/or process improvement initiatives.
The Care Coordinator Lead reports to the Team Manager.
Supervision Exercised:
- Yes, which includes but is not limited to providing leadership, work direction, and performance feedback to subordinate staff.
Essential Duties & Responsibilities:
Provides supervision and leadership to Care Coordinators.
As a subject matter expert (SME), provides guidance and coaching for Care Coordinator tasks.
Accountable to the performance of direct reports.
Reviews documentation and reports to ensure process accuracy and timeliness.
Works collaboratively with the Clinical Care Managers on the team to ensure member needs are being met.
Facilitates team meetings and team huddles.
Other duties as assigned by leadership.
Working Conditions:
Standard office conditions.
Work locations include residential and community sites.
Required Education (must have):
- Associates Degree or 2+ years of applicable experience.
Required Licensing (must have):
- Must have reliable transportation to travel to CCA sites across the state of Massachusetts and to visits in a home, residential, or provider-based setting.
Required Experience (must have):
- 2+ years with a health plan, medical group or health-related facility such as a hospital or outpatient center
Desired Experience (nice to have):
1 + years leadership experience preferred
Managed care experience

Strong leadership and team-building skills
Ability to function independently and effectively and as part of an interdisciplinary team
Ability to manage and lead significant change
Excellent communication skills
Experience using motivational interviewing techniques in a professional setting
Ability to teach, mentor, and coach
Ability to work in a fast-paced environment with changing priorities while maintaining-and fostering in staff and partners- a positive and productive attitude, striving for excellence and continuous quality improvement.
Must be familiar with using a laptop computer as well as Windows processing systems and have knowledge of Microsoft Office, specifically Word, Excel, and Outlook.
Able to support teams with utilization of telehealth technologies (video, chat, etc.), when appropriate, as an approved modality for a variety of clinical care and care management services
