Role Overview :
In partnership with the Capital Planning Senior Project Manager, the School-based Transition Coordinator will work closely with the school leadership team at the Frederick or the West Zone School to develop and maintain a system to support students and families impacted by a school transition due to a closure, merger, or consolidation. As a dedicated team member who knows the school community well, the Coordinator will serve as the primary on-site point of contact for students and families experiencing a transition and will be an integral part of ensuring the best school transition experience possible.
Transition Operations Support
Provide one-on-one transition advisory support and case management to students impacted by a school closure, merger, or consolidation
Collaborate with other student support services teams (Social Workers, Family Liaisons, and Welcome Services team) on student and family transition supports
Maintain student transition files including documented plans
Works with school leader to develop and operationalize packing and moves, including coordinating staff breaks for packing, coverage, and timeline, working within the appropriate union contracts
Community Engagement and Partnerships
Serve as the primary on-site point of contact for students and families, working closely with the Capital Planning Senior Project Manager to address questions and concerns
Coordinate resources and supports for families and students experiencing hardship/challenges due to school closures
As a member of the school transition planning team, work with the Capital Planning Senior Project Manager to help plan and implement transition events for students and families
In partnership with the Capital Planning Senior Project Manager, schedule, coordinate and attend community meetings
Develop information sharing systems across multiple mediums (website, telephone blasts, backpacked materials, etc.)
Meet monthly with the Capital Planning Senior Project Manager in support of planning efforts
Must be available evenings and weekends to support community engagement efforts
Required Qualifications:
